摘  要:目的观察具有泻浊解毒通络作用的萆苓祛痛方(简称萆苓方)对糖尿病痛风大鼠心肌组织内脂素(Visfatin)蛋白表达的调节及对心脏病理的影响。方法选择健康雄性大鼠50只,体质量(180±20)g。除正常组8只外,其余实验模型组[模型组、萆苓祛痛方组(萆苓组)、吲哚美辛组、吡格列酮组]给予高脂饲料喂养,小剂量链脲佐菌素40 mg/kg腹腔注射1次,以血糖≥16.7mmol/L为糖尿病模型。4 d后关节腔注射5%尿酸钠溶液1次,诱导痛风模型。造模成功后,萆苓组、吲哚美辛组及吡格列酮组继续给药21 d,正常组、模型组给予等量生理盐水。3周后取心脏,采用WB法测定心肌组织Visfatin变化,做病理测定。取血测定血糖、血脂、血尿酸。结果与正常组比较,模型组心肌组织Visfatin蛋白表达量明显升高(P<0.01);与模型组比较,萆苓组、吲哚美辛组、吡格列酮组的Visfatin表达量显著下降(P<0.01)。条带图的结果同样显示:与正常组比较,模型组表达亮度明显增强;与模型组比较,药物组的表达亮度显著减弱(P<0.01)。心脏病理提示:正常组心肌组织无炎细胞浸润,无细胞水肿,心肌组织纤维分布均匀,心肌细胞排列有序。模型组有炎细胞浸润,细胞变性水肿,心肌细胞排列无序,肌纤维有嗜酸样变性。萆苓组基本无炎细胞浸润,心肌细胞排列整齐,无细胞水肿。吡格列酮组及吲哚美辛组炎细胞减少,无明显水肿,无心肌纤维变性,细胞排列基本整齐。与正常组比较,模型组血糖(GLU)、血脂(TG、TC、LDL-C)、尿酸(UA)升高(P<0.01);与模型组比较,各药物组血糖、TG、TC、LDL-C、UA显著下降(P<0.01或P<0.05)。萆苓组优于吲哚美辛组(P<0.05)。结论萆苓祛痛方可干预糖尿病痛风大鼠心肌组织Visfatin蛋白的高表达,减轻心肌组织的病理损伤,调节血糖、血脂,降低血尿酸。Objective To observe the regulation of Biling Qutong Decoction on the expression of visfatin protein in myocardial tissue of diabetic gout rats and its influence on cardiac pathology.Methods Fifty healthy male rats were selected and weighed(180±20)g.In addition to the normal group,the model group,biling group,indomethacin group and pioglitazone group were given high-fat diets.The diabetic model was established by intraperitoneal injection of 40 mg/kg streptozotocin with blood sugar≥16.7 mmol/L.After 4 days,the 5%sodium urate solution was injected into the joint cavity once to induce a gout model.After the establishment of model,Biling group,indomethacin group and pioglitazone group continued to be administered for 21 days.The normal group and the model group were given the same amount of normal saline.After 3 weeks,the heart was taken for pathological measurement.Blood was taken to measure blood glucose,lipid and uric acid.Results Compared with the normal group,the expression of visfatin protein in the model group was significantly increased(P<0.01).Compared with the model group,the expression of visfatin in the Biling group,indomethacin group and pioglitazone group decreased significantly(P<0.01);the results of the band chart also showed that the expression brightness of the model group was significantly increased,and the expression brightness of the drug group was significantly weakened(P<0.01);Cardiac pathological examination showed that there was no inflammatory cell infiltration in the normal group,no cell edema,myocardial tissue fiber distribution was uniform,and myocardial cells were arranged neatly.The model group had inflammatory cell infiltration,cell degeneration and edema,disordered myocardial cells,and eosinophilic degeneration of muscle fibers.The sputum group was basically infiltrated,no inflammatory cells,and the myocardial cells were arranged neatly and without cell edema.In the western medicine group,the inflammatory cells were reduced,there was no obvious edema,no myocardial fibrosis,and the
