摘 要:目的观察痹症宁对II型胶原蛋白诱导的类风湿关节炎(Collagen-induced arthritis,CIA)大鼠血清炎症因子C反应蛋白(CRP)、血沉(ESR)以及滑膜中基质金属蛋白酶-3(MMP-3)、基质金属蛋白酶-9(MMP-9)表达的影响,来探讨痹症宁治疗类风湿关节炎(Rrheumatoid arthritis,RA)的作用机制。方法将60只Wistar大鼠随机分为空白对照组10只,其余建立CIA模型,将造模成功的大鼠随机分为模型对照组,痹症宁低、中、高剂量组,甲氨蝶呤组,各组10只,致炎后第14天开始进行药物干预4周。观察各组体质量变化、关节炎指数评分,取大鼠血清检测CRP、ESR含量,ELISA法检测膝关节滑膜中MMP-3、MMP-9表达的水平。结果给药组均可改善CIA大鼠的体质量,其中痹症宁中剂量组、甲氨蝶呤组给药28 d后与模型对照组相比差异显著(P<0.01)。给药2~4周后,各给药组AI积分均有所下降,相比模型对照组有统计学意义(P<0.05或P<0.01)。各给药组大鼠血清CRP、ESR含量对比模型对照组下降明显,差异均有统计学意义(P<0.01);痹症宁中剂量组CRP含量,甲氨蝶呤组CRP、ESR含量下降与痹症宁低剂量组、高剂量组比较,有显著差异性(P<0.01)。在滑膜组织MMP-3、MMP-9水平方面上,各给药组较模型对照组均降低(P<0.05或P<0.01),甲氨蝶呤组下降最显著(P<0.05或P<0.01),痹症宁各剂量组滑膜中MMP-3、MMP-9表达都有所降低,其中痹症宁中剂量组明显降低(P<0.05)。结论痹症宁可改善CIA大鼠一般情况,降低其关节炎指数评分,降低血清炎症因子CRP、ESR以及抑制滑膜组织中MMP-3、MMP-9的表达,具有一定抗炎消肿、调节免疫的作用。Objective To observe the effect of Bizhengning Decoction on serum inflammatory factors C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)and expression of matrix metalloproteinase-3(MMP-3)and MMP-9 in synovial membrane in type II collagen-induced collageninduced arthritis(CIA)rats,to further explore the mechanism of action of the drug.Methods Wistar rats were divided into control group(n=10),and CIA rats,CIA rats were then randomly divided into model group,Bizhengning Decoction(BZND)low,middle and high dose group,methotrexate group.Drug intervention started on the 14 th day after inflammation for 4 weeks.To observe the changes in body mass,arthritis index(AI),serum CRP and ESR were evaluated;To detect the content of MMP-3 and MMP-9 in synovial of RA rats by ELISA.Results The weight of CIA rats was improved in the drug-administered group,and the difference was significant between the BZND middle-dose group and the methotrexate group after 28 days of treatment(P<0.01).AI scores showed that after 2-4 weeks of administration,the AI scores of each group were decreased,which was statistically significant compared with the model control group(P<0.05 or P<0.01).The serum CRP and ESR levels in the control group were significantly lower than those in the model control group(P<0.01).The CRP levels in the BZND middle-dose group and the CRP and ESR levels in the methotrexate group were lower than those in the BZND low-dose and high-dose group.There was a significant difference in the dose group(P<0.01).In the synovial tissue MMP-3,MMP-9 levels,the drug-administered group was lower than the model control group(P<0.05 or P<0.01),and the methotrexate group was the most significant(P<0.05 or P<0.01).The expression of MMP-3 and MMP-9 in the synovial membrane of each BZND dose group was decreased,and the BZND middle-dose group was significantly decreased(P<0.05).Conclusion Bizhengning Decoction can improve the general condition of CIA rats,reduce its arthritis index score,reduce serum inflammatory factors CRP,ESR and inhib
