显微镜下多血管炎合并2型糖尿病并发脓肿感染的护理分析

摘　　要：目的分析封闭负压吸引术与血糖管理在以四肢脓肿为主要临床表现的显微镜下多血管炎合并2型糖尿病患者中的应用效果,总结护理措施,以更好的指导临床工作。方法选取2016年1月~2017年9月于我院接受治疗的显微镜下血管炎合并2型糖尿病患者5例,均给予皮肤护理、疼痛评估、血糖安全管理及个性化饮食指导等护理措施,观察患者预后。结果1例患者因放弃治疗自动出院,其余4例无新发皮疹,无肌痛、血糖控制尚可,脓肿切开引流彻底,二期愈合。结论血糖安全管理是围手术期关注重点,封闭负压吸引可有效促进创面愈合,疼痛评估、皮肤护理及饮食指导等积极有效护理干预是保证显微镜下多血管炎合并糖尿病并发脓肿患者治疗效果的重要环节。Objective To analyze the effect of closed negative pressure aspiration and blood sugar management in patients with type 2 diabetes mellitus and polyangiitis with microscopic abscesses as the main clinical manifestations,and to summarize nursing measures to better guide clinical work.Methods 5 patients with microscopic vasculitis and type 2 diabetes mellitus who were treated in our hospital from January 2016 to September 2017 were selected,all were given skin care,pain assessment,blood sugar safety management and personalized dietary guidance,and observe the patient's prognosis.Results 1 patient was discharged from the hospital due to abandonment of treatment,the remaining 4 patients had no new rashes,no myalgia,and glycemic control was acceptable,the abscess was completely incised and drained,and the second stage healed.Conclusion The safe management of blood glucose is the focus of attention during the perioperative period,the closed negative pressure attraction can effectively promote wound healing,active and effective nursing interventions such as pain assessment,skin care and diet guidance are important links to ensure the treatment effect of patients with polyangiitis combined with diabetes and abscess under the microscope.
