Ponseti法治疗30例先天性马蹄内翻足患者的临床分析

摘　　要：目的探讨Ponseti法治疗先天性马蹄内翻足患者的临床疗效。方法选择我院2013年9月~2018年5月收治的59例患者随机分为观察组(n=30)和对照组(n=29)。对照组给予传统治疗,观察组给予Ponseti法治疗。观察比较两组临床疗效、Dimeglio评分及复发情况。结果观察组痊愈7例(23.33%)、显效16例(53.33%)、有效5例(16.67%),临床疗效优于对照组,两组差异有统计学意义(Z=-2.041,P=0.041)。治疗后,两组Dimeglio评分与治疗前比较均显著下降,差异有统计学意义(t=22.028,P=0.000;t=14.087,P=0.000);且治疗后观察组Dimeglio评分(1.34±0.55)分显著低于对照组(2.91±0.67)分,差异有统计学意义(t=-9.820,P=0.000)。观察组复发1例(3.33%),明显低于对照组的7例(24.14%),差异有统计学意义(P=0.026)。结论Ponseti法治疗CCF临床疗效显著,可有效降低Dimeglio评分,减少临床复发,促进患儿足部恢复,值得推广应用。Objective To explore the clinical effect of Ponseti method in patients with congenital clubfoot(CCF).Methods 59 patients admitted to our hospital from September 2013 to May 2018 were randomly divided into observation group(n=30)and control group(n=29).The control group was given traditional treatment,while the observation group was given Ponseti treatment.The clinical efficacy,Dimeglio score and recurrence were observed and compared between the two groups.Results There were 7 cases(23.33%)cured,16 cases(53.33%)markedly effective,and 5 cases(16.67%)effective in the observation group.The clinical efficacy in the observation group was better than that of the control group,and the difference was statistically significant(Z=-2.041,P=0.041).After treatment,the Dimeglio score in both groups was significantly lower than those before treatment,and the difference was statistically significant(t=22.028,P=0.000;t=14.087,P=0.000);And the Dimeglio score in the observation group after treatment was(1.34±0.55)points,which was significantly lower than(2.91±0.67)points in the control group,and the difference was statistically significant(t=-9.820,P=0.000).There was one case(3.33%)of recurrence in the observation group,which was significantly lower than 7 cases(24.14%)in the control group,and the difference was statistically significant(P=0.026).Conclusion Ponseti method has a significant clinical effect on CCF,which can effectively decrease the Dimeglio score,reduce clinical recurrence,and promote foot recovery in children.It is worth promoting and applying.
