摘 要:目的探讨骨水泥弥散类型与PVP术后早期镇痛效果及延迟性椎体高度丢失的相关性。方法对本科2016年1月-2018年10月采用PVP手术治疗的81例OVCF患者进行观察,依据术后骨水泥弥散等级分为三组:骨水泥弥散等级为1级26例,纳入1组;2级36例,纳入2组;3级19例,纳入3组。对三组患者的术后3 d、术后1年VAS评分及延迟性高度丢失情况进行比较;同时,对骨水泥弥散等级与VAS评分、延迟性高度丢失值进行Spearman相关性分析。结果术后3 d,三组VAS评分均较术前显著降低(P<0.05),但1组>2组>3组,差异有统计学意义(P<0.05);三组术后1年的VAS评分差异无统计学意义(P>0.05)。术后1年,三组患者均出现伤椎高度丢失现象,但1组>2组>3组,差异有统计学意义(P<0.05)。经Spearman相关性分析,骨水泥弥散等级与术后3 d VAS评分、椎体延迟性高度丢失值呈负相关关系(P<0.05)。结论骨水泥弥散等级越高,则PVP术后的即时镇痛效果越好、椎体高度丢失也越轻微。Objective To investigate the correlation between the diffusion type of bone cement and the early analgesic effect and delayed vertebral height loss after PVP.Methods The clinical data of 81 osteoporotic vertebral compression fracture(OVCF)patients treated by PVP from January 2016 to October 2018 in our department were analyzed retrospectively.According to the grade of bone cement diffusion after operation,they were divided into three groups:26 patients with grade 1 bone cement diffusion were included in group 1;36 patients with grade 2 bone cement diffusion were included in group 2;19 patients with grade 3 were included in group 3.The VAS score,vertebral height and delayed height loss of three groups were compared at 3 d and 1 year after operation.Meanwhile,the correlation between bone cement diffusion grade,VAS score and delayed height loss was analyzed by Spearman analysis.Results On the third day after operation,the VAS scores of the three groups were significantly lower than before operation(P<0.05),and group 1>group 2>group 3,the difference was statistically significant(P<0.05).There was no significant difference in VAS score among the three groups at 1 year after operation(P>0.05).At 1 year after operation,the height of the injured vertebrae was lost in all three groups,but group 1>group 2>group 3,the difference was statistically significant(P<0.05).According to Spearman's correlation analysis,there was a negative correlation between the grade of bone cement diffusion and VAS score and delayed height loss of vertebral body after operation(P<0.05).Conclusions The higher the diffusion grade of bone cement,the better the immediate analgesic effect and the less loss of vertebral height after PVP.
