髓内钉固定治疗老年性股骨粗隆间骨折的效果分析

摘　　要：目的:分析髓内钉固定对老年性股骨粗隆间骨折的治疗效果。方法:选取2017年3月—2019年6月于我院就诊的56例老年股骨粗隆间骨折患者,随机分为对照组与观察组,各28例。对照组行动力髋螺钉固定,观察组行髓内钉固定。对比分析两组手术一般情况、疗效及生活质量。结果:观察组手术时间85.66±18.38m in、切口长度5.70±1.12cm、出血量220.12±43.45ml、骨折愈合时间11.54±1.19周、住院时间7.76±2.31d均低于对照组97.30±20.67min、10.45±2.44cm、339.57±57.83ml、13.66±1.54周、13.43±2.46d(P<0.05);观察组治疗总有效率96.43%高于对照组的78.57%(P<0.05);观察组生理职能评分71.20±3.13、生理功能65.33±3.72、情感职能75.21±2.61、精神状态67.76±2.30、社会功能69.22±3.40、高于对照组的52.01±3.10、55.11±2.95、55.21±2.76、52.76±2.40、53.27±2.60(P<0.05)。结论:对老年股骨粗隆间骨折患者采取髓内钉固定疗效显著,可有效改善患肢功能,缩短治疗及骨折愈合时间,提升生活质量,值得应用。Objective To explore the effect of intramedullary nail fixation on the elderly intertrochanteric fracture and the quality of life of the patients.Methods 56 elderly patients with intertrochanteric fracture of femur treated in our hospital from March 2017 to June 2019 were randomly divided into control group and observation group,28 cases in each group.In the control group,hip screw fixation was used,while in the observation group,intramedullary nail fixation was used.The general situation,curative effect and quality of life of the two groups were compared.Results The operation time,incision length,bleeding volume,fracture healing time and hospitalization time of the observation group were 85.66±18.38min,5.70±1.12cm,220.12±43.45ml,11.54±1.19 weeks,7.76±2.31d,respectively,which were lower than those of the control group(97.30±20.67min,10.45±2.44cm,339.57±57.83ml,13.66±1.54 weeks,13.43±2.46d,P<0.05);the total effective rate of the observation group was 96.43%higher than that of the control group(85.71,P<0.05);the birth rate of the observation group was higher than that of the control group(P<0.05)physiological function 71.20±3.13,Physical function score 65.33±3.72,emotional function 75.21±2.61,social function 69.22±3.40,mental state 67.76±2.30 were higher than the control group 52.01±3.10,55.11±2.95,55.21±2.76,52.76±2.40,53.27±2.60(P<0.05).Conclusion Intramedullary nail fixation is effective in the elderly patients with intertrochanteric fracture of femur.It can effectively avoid cubitus varus deformity,shorten the time of treatment and callus healing,and improve the quality of life.It is worth popularizing.
