摘 要:目的探讨超声测量仰卧位舌颏距离率(HMDR)对壮族人群困难气道的预测价值。方法选择择期行全麻气管插管手术治疗的171例壮族患者。麻醉前采用超声定位舌骨,测量并记录患者的仰卧中立位舌颏距离(HMDn)与过伸位舌颏距离(HMDe),并计算HMDR。麻醉诱导后普通喉镜暴露下气管插管,根据C/L分级诊断困难气道。评估HMDR对困难气道的预测价值。结果壮族患者的困难气道发生率为12.87%(22/171)。困难气道患者与非困难气道患者的仰卧位HMDn差异均无统计学意义(均P>0.05),而非困难气道患者的仰卧位HMDe及HMDR长于或高于困难气道患者(均P<0.05)。受试者工作特征曲线结果显示,仰卧位HMDe、HMDn、HMDR的预测壮族患者困难气道的曲线下面积分别为0.692、0.505、0.812,其中仰卧位HMDR的曲线下面积大于仰卧位HMDn(P<0.05),且灵敏性最高。结论超声测量仰卧位HMDR对评估壮族患者困难气道有较高的预测价值。Objective To explore the predictive value of ultrasonic measurement of hyomental distance rate(HMDR)in the supine position for difficult airway in Zhuang people.Methods A total of 171 patients of Zhuang nationality scheduled for endotracheal intubation under general anesthesia were selected.Before anesthesia ultrasonic positioning of hyoid bone was performed to measure and record hyomental distance in the neutral position(HMDn)and hyomental distance at the extreme of head extension(HMDe)in the supine position,and HMDR was calculated.After anesthesia induction endotracheal intubation was performed under general laryngoscopic exposure,difficult airway was diagnosed according to C/L classification.The predictive value of HMDR for difficult airway was evaluated.Results The incidence rate of difficult airway in Zhuang patients was 12.87%(22/171).There was no statistically significant difference in HMDn in the supine position between patients with difficult airway and patients with non-difficult airway(all P>0.05),and patient with non-difficult airway had longer HMDe and higher HMDR in the supine position than patients with difficult airway(all P<0.05).Receiver operating characteristic curve results showed that the areas under the curve of HMDe,HMDn and HMDR in the supine position for predicting difficult airway were 0.692,0.505 and 0.812,respectively,and HMDR in the supine position had greater area under the curve than HMDn in the supine position(P<0.05),exhibiting the highest sensitivity.Conclusion Ultrasonic HMDR measurement in the supine position has a higher predictive value for the evaluation of difficult airway in Zhuang patients.
