摘　　要：目的比较输尿管软镜碎石取石术(FURL)和经皮肾镜碎石取石术(PCNL)治疗一期PCNL术后肾结石残余患者的疗效及安全性。方法回顾性分析114例一期PCNL术后肾结石残余患者的临床资料,根据手术方法将患者分为对照组64例与观察组50例,对照组采用PCNL治疗,观察组采用FURL治疗。比较两组患者残石彻底清除率、手术相关指标及术后并发症发生率。结果两组残石彻底清除率、术后并发症发生率差异均无统计学意义(均P>0.05),观察组术中出血量、术后住院时间、肾造瘘管留置时间均少于或短于对照组,而手术时间和治疗费用均长于或多于对照组(均P<0.05)。结论采用FURL治疗一期PCNL术后肾结石残余患者在降低术中创伤程度和加快患者术后康复进程方面更有优势,而PCNL则在缩短手术用时和降低经济负担方面更有优势。Objective To compare the efficacy and safety of flexible ureteroscopic lithotripsy(FURL)versus percutaneous nephrolithotomy(PCNL)in the treatment of patients with residual renal calculi after stageⅠPCNL.Methods The clinical data of 114 patients with residual renal calculi after stageⅠPCNL were retrospectively analyzed,and patients were divided into control group(64 cases)and observation group(50 cases)according to the surgical approach.The control group was treated with PCNL,whereas the observation group was treated with FURL.The rate of complete removal of residual stones,operation-related indexes,and incidence rate of postoperative complications were compared between the two groups.Results There was no statistically significant difference between the two groups in rate of complete removal of residual stones or incidence rate of postoperative complications(all P>0.05),the observation group had less intraoperative blood loss,shorter postoperative hospital stay,shorter duration of nephrostomy tube retention,longer operation duration and more treatment costs as compared with the control group(all P<0.05).Conclusion For patients with residual renal calculi after stageⅠPCNL,FURL has more advantages in reducing the severity of intraoperative trauma and advancing the process of patients′postoperative recovery,while PCNL has more advantages in shortening operation duration and diminishing economic burden.
