摘  要:目的探讨丹参水提物和醇提物对顺铂致肾损伤大鼠肾功能的保护作用及其机制。方法将60只SD大鼠随机分为空白组、顺铂组、氨磷汀对照组、丹参水提取物组、丹参醇提取物组,每组12只。采用尾静脉注射顺铂法建立顺铂致肾损伤大鼠模型。建模后第1天空白组、顺铂组给予生理盐水灌胃,其他3组腹腔注射相应药物,连续给药21 d。给药结束后,检测5组大鼠尿肾损伤因子1(KIM-1)、尿N-乙酰-β-D-氨基葡萄糖苷酶(NAG)和24 h尿蛋白,血肌酐、尿素氮、胱抑素C、乳酸脱氢酶(LDH),肾脏组织谷胱甘肽过氧化物酶(GSH-Px)、一氧化氮、超氧化物歧化酶(SOD)、丙二醛水平及活性氧簇(ROS)含量及KIM-1、转化生长因子β1 mRNA表达水平。结果与空白组比较,各组大鼠24 h尿蛋白、尿NAG、KIM-1,血肌酐、尿素氮、胱抑素C水平、LDH活性,肾组织丙二醛、一氧化氮、ROS水平升高,而肾组织SOD、GSH-Px水平降低(均P<0.05)。与顺铂组比较,氨磷汀对照组、丹参水提取物组、丹参醇提取物组大鼠24 h尿蛋白、尿NAG、KIM-1水平,血肌酐、尿素氮、胱抑素C水平、LDH活性,肾组织丙二醛、一氧化氮、ROS水平均降低,而肾组织SOD、GSH-Px水平升高(均P<0.05)。与氨磷汀对照组和丹参水提取物组比较,丹参醇提取物组大鼠24 h尿蛋白、尿NAG、KIM-1水平,血肌酐、尿素氮、胱抑素C水平、LDH活性,肾组织丙二醛、一氧化氮、ROS水平均降低,而肾组织SOD、GSH-Px水平均升高(均P<0.05)。与空白组比较,除丹参醇提取物组外,其他各组大鼠肾组织KIM-1 mRNA表达水平升高(均P<0.05);与顺铂组比较,氨磷汀对照组、丹参水提取物组、丹参醇提取物组大鼠KIM-1 mRNA表达水平降低(均P<0.05);氨磷汀对照组与丹参水提取物组大鼠KIM-1 mRNA表达水平高于丹参醇提取物组(均P<0.05)。结论丹参水提物和醇提物均能改善顺铂致肾损伤大鼠的肾功能,且醇提物效果更好,其�Objective To investigate the protective effects of water extract and alcohol extract of Salvia miltiorrhiza on renal function and its mechanism in rats with cisplatin-induced kidney injury.Methods Sixty SD rats were randomly divided into blank group,cisplatin group,amifostine control group,water extract of Salvia miltiorrhiza group,alcohol extract of Salvia miltiorrhiza group,with 12 rats in each group.The rat model of cisplatin-induced kidney injury was established by tail intravenous injection of cisplatin.On the first day of remodeling,the blank group and the cisplatin group were given normal saline by gavage for 21 consecutive days,and the other three groups were given peritoneal injection of corresponding drugs for 21 consecutive days.After the medication was completed,the indices of the rats in the five groups were detected,including the levels of kidney injury molecule 1(KIM-1),urinary N-acetyl-beta-D-amino glycosidase enzymes(NAG),24-hour urine protein,serum creatinine,blood urea nitrogen(BUN),cystatin C and lactate dehydrogenase(LDH),the levels of kidney tissue glutathione peroxidase(GSH-Px),nitric oxide,superoxide dismutase(SOD)and malondialdehyde,reactive oxygen species(ROS)content,as well as the expression levels of KIM-1 and transforming growth factorβ1 mRNAs.Results Compared with the blank group,rats in the other groups had elevated levels of 24-hour urine protein,urinary NAG,KIM-1,serum creatinine,BUN and cystatin C,increased LDH activity,elevated levels of kidney tissue malondialdehyde,nitric oxide and ROS,and decreased levels of kidney tissue SOD and GSH-Px(all P<0.05).Compared with the cisplatin group,rats in the amifostine control group,the water extract of Salvia miltiorrhiza group and the alcohol extract of Salvia miltiorrhiza group exhibited decreased levels of 24-hour urine protein,urinary NAG and KIM-1,lower levels of serum creatitine,BUN and cystatin C,decreased LDH activity,decreased levels of kidney tissue malondialdehyde,nitric oxide and ROS,and higher levels of kidney tissue SOD and
