北京地区变应性鼻炎患者血清总IgE及sIgE结果分析

摘　　要：目的分析北京地区变应性鼻炎(AR)患者血清总免疫球蛋白E(IgE)及特异性IgE(sIgE)水平检测结果,总结其主要变应原种类,以及主要变应原随月份的变化情况,为该地区AR患者的预防、诊断及治疗提供依据。方法选取2018年10月至2019年10月于解放军总医院耳鼻咽喉头颈外科就诊的AR患者789例,采用德国欧蒙EUROIMMUN公司生产的Euroline中国特应性组合20种标准变应原试剂,测试AR患者血清中总IgE和变应原sIgE水平。结果789例AR患者年龄高峰为23~41岁,其中总IgE阳性患者616例(78.07%),sIgE阳性患者644例(81.62%)。常见吸入性过敏原中,尘螨组合,树木组合3,和屋尘为主要的过敏原,分别占AR患者的41.19%(325例),29.66%(234例),24.46%(193例)。食入性过敏原中,淡水鱼组合和海鲜组合为主要过敏原,分别占AR患者的28.90%(228例)和23.70%(187例)。北京地区AR患者发病时间高峰为3~5月和7~9月。在主要吸入性变应原中,常年性变应原(尘螨组合,屋尘和动物毛发组合)血清sIgE水平均值随月份波动较一致,季节性变应原(树组合,艾蒿)血清sIgE水平均值随月份波动与各自的开花季节一致。结论北京地区AR患者以青壮年为主,尘螨组合,树木组合3,和屋尘为主要的吸入性过敏原,淡水鱼组合和海鲜组合为主要食入性过敏原。在每年的3~5月和7~9月,榆树、梧桐树、柳树、杨树、柏树和艾蒿等常见植物开花季节发病率最高,AR患者应注意提前防护。Objective To analyze the detection results of serum total immunoglobulin E(IgE)and specific IgE(sIgE)in patients with allergic rhinitis(AR)in Beijing area,and to summarize the main allergen types and the major allergens with monthly pattern.The results will provide a knowledge basis for the prevention,diagnosis and treatment of AR patients in this area.Methods A total of 789 patients with AR who were treated in the Department of Otorhinolaryngology Head and Neck Surgery of the General Hospital of the Chinese People′s Liberation Army from October,2018 to October,2019 were selected for the study,and 20 standard allergen reagents from Euroline produced by EUROIMMUN were used to test AR patients′serum levels of total IgE and allergen sIgE.Results The peak age range of 789 AR patients was 23-41 years old,including 616(78.07%)total IgE positive patients and 644(81.62%)sIgE positive patients.Among the common inhaled allergens,dust mite combination,tree combination 3,and house dust were the main allergens,accounting for 41.19%(325 cases),29.66%(234 cases),and 24.46%(193 cases)of AR patients,respectively.Among ingestive allergens,the freshwater fish combination and seafood combination were the main allergens,accounting for 28.90%(228 cases)and 23.70%(187 cases)of AR patients,respectively.The peak incidence of AR patients in Beijing area was identified during months of March-May and July-September.Among the main inhaled allergens,the average serum sIgE concentration of perennial allergens(dust mite combination,house dust and animal hair combination)fluctuates with a pattern consistent with months,while seasonal allergens(tree combination,mugwort)serum sIgE had their mean concentrations fluctuated with the month and were consistent with their individual flowering seasons.Conclusion AR patients in Beijing area are mainly young adults.The dust mite combination,the tree combination 3,and the house dust are the main inhaled allergens,and the freshwater fish combination and the seafood combination are the main ingested aller
