摘　　要：目的 研究多层螺旋CT联合24小时动态心电图在冠心病诊断中的应用效果.方法 选取2016年12月至2019年11月入院的180例疑似冠心病患者.所有患者均进行多层螺旋CT、24小时动态心电图、冠状动脉造影检查.以冠状动脉造影结果 作为诊断金标准,比较多层螺旋CT、24小时动态心电图单独检查及联合检查的诊断效果.结果 180例患者中,经冠脉动脉造影证实104例阳性,76例阴性.多层螺旋CT联合24小时动态心电图诊断冠心病的准确率、阴性预测值、阳性预测值、特异度、灵敏度及冠心病罪犯血管定位准确率均高于24小时动态心电图和多层螺旋CT单独检查(P<0.05).结论 多层螺旋CT联合24小时动态心电图在冠心病诊断中的应用效果显著,可进一步提升冠心病诊断准确率、灵敏度、特异度及罪犯血管定位准确性,减少误诊.Objective To study the application effect of multislice spiral CT combined with 24-hour dynamic electrocardiogram in the diagnosis of coronary heart disease.Methods One hundred and eighty patients suspected coronary heart disease admitted in our hospital from December 2016 to November 2019 were selected.All patients underwent multislice spiral CT,24-hour dynamic electrocardiogram and coronary angiography.The results of coronary angiography were regarded as the gold standard to compare the diagnostic effects of multislice spiral CT and 24-hour dynamic electrocardiogram alone and combined examination.Results Among 180 patients,104 cases were positive and 76 cases were negative diagnosed by coronary angiography.The accuracy rate,negative predictive value,positive predictive value,specificity,sensitivity and the accuracy rate of culprit vessel location of coronary heart disease diagnosed by multislice spiral CT combined with 24-hour dynamic electrocardiogram were higher than those of multislice spiral CT and 24-hour dynamic electrocardiogram alone examination(P<0.05).Conclusion Multislice spiral CT combined with 24-hour dynamic electrocardiogram in the diagnosis of coronary heart disease has a significant effect.It can further improve the accuracy,sensitivity,specificity and the accuracy rate of culprit vessel location of coronary heart disease,and reduce misdiagnosis.
