摘　　要：目的:建立临床药师抗感染会诊意见模板及评价体系,实现对临床药师抗感染会诊工作的标准化。方法:基于我院的会诊模式和贵州省药事管理质控中心的平台优势,采用头脑风暴法、问卷调查法和焦点小组访谈法逐步建立临床药师抗感染会诊意见的模板及评价体系。结果:头脑风暴法建立的初稿包括会诊意见模板一(8个条目)和模板二(9个条目),问卷调查后调整构建了会诊意见的评价体系(模板一7个条目28.0分和模板二8个条目36.0分),焦点小组访谈法对其中10个条目进行了修改。结论:建立的会诊意见模板及评价体系对我省临床药师抗感染会诊工作提出了新的要求,其内容还需实证研究进一步验证和完善。OBJECTIVE To establish the template and evaluation system of anti-infection consultation opinions of clinical pharmacists,and to realize the standardization of anti-infection consultation of clinical pharmacists.METHODS Based on the consultation mode of our hospital and the platform of the Quality Control Center of Guizhou Pharmacy Administration,brainstorming method,questionnaire survey method and focus group discussion were used to gradually establish the template and evaluation system of anti-infection consultation opinions of clinical pharmacists.RESULTS The draft of the template of consultation opinions included two parts:8 items for Part I and 9 items for PartⅡ.Then the adjusted templates and evaluation system of consultation opinions were developed,including 7 items for Part I with a total score of 28.0 points and 8 items for PartⅡwith a total score of 36.0 points.Furthermore,10 items were revised by focus group discussion.CONCLUSION The established consultation opinion template and evaluation system put forward new requirements for anti-infection consultation of clinical pharmacists in our province,and its content requires further verification and improvement by empirical studies.
