自体富血小板凝胶治疗糖尿病皮肤慢性难愈合创面的临床疗效

摘　　要：目的探讨自体富血小板凝胶(APG)治疗糖尿病皮肤慢性难愈合创面的临床疗效。方法选取广东省高州市慢性病防治站皮肤科和广东省江门市新会区第二人民医院皮肤科2016年9月—2018年6月收治的糖尿病皮肤慢性难愈合创面患者138例,按照随机数字表法分为对照组与观察组,各69例。对照组予以重组牛碱性成纤维细胞生长因子凝胶治疗,观察组予以APG治疗。两组均至少连续治疗1个月。比较两组临床疗效,溃疡愈合指标(溃疡面愈合时间、换药次数),治疗第7天、14天、21天和28天创面闭合指数。结果观察组治疗总有效率高于对照组,溃疡面愈合时间短于对照组,换药次数少于对照组(P<0.05)。治疗第7天两组创面闭合指数比较,差异无统计学意义(P>0.05);治疗第14天、21天和28天观察组创面闭合指数高于对照组(P<0.05)。两组治疗期间均未发生明显不良反应。结论APG治疗糖尿病皮肤慢性难愈合创面的临床疗效确切,可有效提高创面闭合指数,缩短愈合时间。Objective To investigate the clinical efficacy of autologous platelet rich gel for chronic refractory wounds of diabetic skin.Methods A total of 138 cases of patients with diabetic skin chronic refractory wound were selected from September 2016 to June 2018 in the Department of Dermatology,Gaozhou Chronic Disease Prevention and Control Station and the Department of Dermatology,the Second People’s Hospital of Xinhui District Jiangmen City,which were divided into control group and research group according to random number table,69 cases each of group.The control group was treated with recombinant bovine basic fibroblast growth factor gel,and the observation group was treated with APG.Both groups were treated for at least one month.The clinical effect,ulcer healing index(ulcer surface healing time,dressing change times),wound closure index on the 7 th,14 th,21 st and 28 th day of treatment were compared between the two groups.Results The total effective rate of the observation group was higher than the control group,the healing time of ulcer surface was shorter than the control group,and the frequency of dressing change was less than the control group(P<0.05).There was no significant difference in the wound closure index between the two groups on the 7 th day(P>0.05);the wound closure index of the observation group was higher than the control group on the 14 th,21 st and 28 th day(P<0.05).There was no significant adverse reactions in the two groups during the treatment.Conclusion The clinical effect of APG in the treatment of diabetic skin chronic refractory wound is definite,it can effectively improve the wound closure index and shorten the healing time.
