摘　　要：目的探讨ω-3鱼油脂肪乳联合早期肠内营养对重型颅脑创伤病人肠道功能及近期预后的影响。方法 2017年3月~2019年3月我院收治的重型颅脑创伤病人98例,用随机数字表法随机分为对照组与观察组2组,每组各49例。对照组采用早期肠内营养治疗,观察组在对照组基础上结合ω-3鱼油脂肪乳治疗。两组疗程均为2周。比较两组治疗前后格拉斯哥昏迷指数评分(GCS)、神经功能缺损程度(NIHSS)和急性生理和慢性健康状况评分(APACHEⅡ)和肠道黏膜屏障功能变化及近期预后情况。结果观察组治疗后GCS评分高于对照组,差异有统计学意义(P<0.05)。观察组治疗后NIHSS评分和APACHEⅡ评分低于对照组,差异有统计学意义(P<0.05)。观察组治疗后血清D-乳酸和内毒素水平低于对照组,差异有统计学意义(P<0.05)。观察组预后良好(79.59%)优于对照组(51.02%),差异有统计学意义(P<0.05)。结论ω-3鱼油脂肪乳联合早期肠内营养对重型颅脑创伤病人效果良好,可改善病人肠道黏膜屏障功能,近期预后良好。Objective To investigate the effect of ω-3 fish oil fat emulsion combined with early enteral nutrition on intestinal function and short-term prognosis in patients with severe craniocerebral trauma.Methods The 98 patients with severe craniocerebral trauma admitted to our hospital from March 2017 to March 2019 were randomly divided into control group and observation group,49 cases in each group.The patients in the control group were treated with early enteral nutrition,while the patients in the observation group were treated with ω-3 fish oil fat emulsion on the basis of the control group.Both groups were treated for 2 weeks.The Glasgow coma index score(GCS),neurological deficit degree(NIHSS),acute physiological and chronic health status score(APACHE II),intestinal mucosal barrier function and short-term prognosis were compared between the two groups before and after treatment.Results Affter treatment,the GCS score of the observation group was higher than that of the control group(P<0.05).After treatment,the NIHSS and APACHE II score of the observation group was lower than that of the control group(P<0.05).The levels of serum D-lactic acid and endotoxin in the observation group were lower than those in the control group after treatment(P<0.05).The prognosis of the observation group was better(79.59%)than that of the control group(51.02%)(P<0.05).Conclusion ω-3 fish oil fat emulsion combined with early enteral nutrition has a good effect on severe craniocerebral trauma patients,can improve the intestinal mucosal barrier function,and has a good prognosis in the near future.
