摘　　要：目的探索14种肌炎抗体与皮肌炎(DM)/多发性肌炎(PM)并发间质性肺疾病(ILD)和恶性肿瘤的相关性。方法采用免疫印迹法筛选149例DM/PM患者的14种肌炎抗体,并对其临床资料进行回顾性分析。结果149例患者中DM 145例,PM 4例。男46例,女103例。高分辨CT诊断显示有71例(47.7%)诊断为ILD,78例(52.3%)未诊断ILD。DM/PM患者中发现恶性肿瘤15例,发生率为10.1%。通过二元Logistic回归分析发现,女性、MDA-5抗体及Ro-52抗体是DM/PM发生ILD的危险因素;Ro-52抗体对DM/PM并发ILD的影响仅次于MDA-5抗体;高龄及TIF1-γ抗体是DM/PM患者并发恶性肿瘤的危险因素。结论女性、MDA-5抗体及Ro-52抗体是DM/PM并发ILD的危险因素。高龄及TIF1-γ抗体是DM并发恶性肿瘤的危险因素。Objective To explore the correlation between 14 kinds of myositis antibodies and interstitial lung disease(ILD)and malignant tumors in dermatomyositis(DM)/polymyositis(PM).Methods Immunoblotting was used to screen 14 kinds of myositis antibodies in 149 patients with DM/PM,and the clinical data were analyzed retrospectively.Results Among the 149 patients,145 were dermatomyositis and 4 were polymyositis.There were 46 males and 103 females.According to the imaging features of high resolution CT,71 cases(47.7%)were diagnosed as ILD.Malignant tumors were found in 15 patients,and the incidence was 10.1%.By binary logistic regression analysis,it was found that female,MDA-5 antibody and Ro-52 antibody were the risk factors for ILD in DM/PM.The influence of Ro-52 antibody on ILD in DM/PM was second only to that of MDA-5 antibody.Old age and TIF1-γantibody were the risk factors of DM/PM complicated with malignant tumor.Conclusions Female,MDA-5 antibody and Ro-52 antibody are risk factors for ILD in DM/PM.Old age and TIF1-γantibody are risk factors for DM patients complicated with malignant tumors.
