宫颈钳夹术联合卡前列素氨丁三醇治疗产后宫缩乏力出血疗效观察

摘　　要：目的探讨宫颈钳夹术联合卡前列素氨丁三醇注射液治疗产后宫缩乏力出血的临床疗效.方法将85例产后宫缩乏力出血患者按治疗方案不同分为两组.对照组(42例)予以宫颈钳夹术治疗,研究组(43例)予以宫颈钳夹术联合卡前列素氨丁三醇注射液治疗.统计并比较两组止血时间、2 h及24 h出血量、凝血功能(凝血酶原时间、活化部分凝血活酶时间、凝血酶时间、纤维蛋白原水平)及并发症发生状况.结果研究组止血时间显著短于对照组(P<0.01),2 h、24 h出血量显著少于对照组(P<0.01),并发症发生率显著高于对照组(P<0.01).治疗后研究组凝血酶原时间、活化部分凝血活酶时间、凝血酶时间短于对照组(P<0.01),纤维蛋白原水平显著高于对照组(P<0.01).结论宫颈钳夹术联合卡前列素氨丁三醇注射液治疗产后宫缩乏力出血能有效改善凝血功能,缩短止血时间,并发症轻微,安全性高.Objective To explore the clinical effect of cervical clamp combined with carboprost tromethamine injection in the treatment of postpartum uterine inertia hemorrhage.Methods 85 patients with uterine inertia hemorrhage were selected and divided into two groups according to different treatment plans.The control group(42 cases)was treated with cervical clamp,and the study group(43 cases)was treated with cervical clamp combined with carboprost tromethamine injection.Hemostasis time,blood loss at 2 and 24 hours after treatment,coagulation function[prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(Fib)level]and complications were compared between the two groups.Results The hemostasis time of the study group was significantly shorter than that of the control group(P<0.01),and the bleeding volume at 2 and 24 hours was significantly less than that of the control group(P<0.01),and the incidence of complications was significantly higher than that of the control group(P<0.01).PT,APTT and TT in the study group were shorter than that of the control group(P<0.01),and Fib was higher than that of the control group(P<0.01).Conclusions Cervical clamp combined with carboprost tromethamine injection in the treatment of postpartum uterine inertia hemorrhage can effectively improve the coagulation function,shorten the hemostasis time,which also has minor complications and high safety.
