戈舍瑞林联合左炔诺孕酮宫内节育器治疗子宫内膜不典型增生疗效观察

摘　　要：目的探讨戈舍瑞林联合左炔诺孕酮宫内节育器治疗子宫内膜不典型增生患者的临床疗效.方法将86例子宫内膜不典型增生患者按随机数字表法分为两组,每组43例.对照组予以戈舍瑞林治疗,研究组予以戈舍瑞林联合左炔诺孕酮宫内节育器治疗,观察12周.比较两组疗效、不良反应发生率、治疗前后子宫内膜厚度、月经量及血红蛋白水平.结果研究组总有效率显著高于对照组(P<0.05).治疗前两组各项指标比较差异无统计学意义(P>0.05);治疗后研究组子宫内膜厚度显著薄于对照组(P<0.01),月经失血图评分显著低于对照组(P<0.01),血红蛋白水平显著高于对照组(P<0.01).结论戈舍瑞林联合左炔诺孕酮宫内节育器治疗子宫内膜不典型增生疗效显著,能明显改善子宫内膜厚度,减少月经量,提高患者血红蛋白水平,优于单用戈舍瑞林治疗.Objective To observe the efficacy of goserelin combined with levonorgestrel intrauterine device in the treatment of endometrial atypical hyperplasia.Methods 86 patients with AEH were randomly divided into two groups,43 cases each.The control group was treated with goserelin,and the study group was treated with goserelin combined with levonorgestrel intrauterine device,and observed for 12 weeks.The curative effect,incidence of adverse reactions,endometrial thickness,menstrual volume(PBAC score)and hemoglobin(Hb)were observed and compared between the two groups before and after treatment.Results The total effective rate of the study group was significantly higher than that of the control group(P<0.05).There was no significant difference in the indexes of the two groups before treatment(P>0.05).After treatment,the endometrial thickness of the study group was significantly thinner than that of the control group(P<0.01),the PBAC score was significantly lower than that of the control group(P<0.01),and the hemoglobin level was significantly higher than that of the control group(P<0.01).Conclusions Goserelin combined with levonorgestrel intrauterine device in the treatment of AEH can significantly enhance the therapeutic effect,improve endometrial thickness,reduce menstrual volume and increase hemoglobin content,better than goserelin alone.
