郭氏仰卧拔伸牵引手法治疗神经根型颈椎病78例临床观察

摘　　要：目的观察郭氏仰卧拔伸牵引手法治疗神经根型颈椎病的临床疗效。方法将156例神经根型颈椎病患者采用随机数字表法分为对照组和治疗组,各78例,对照组予常规手法及牵引治疗,治疗组予郭氏仰卧拔伸牵引手法治疗。2组均隔日治疗1次,治疗7次后比较2组的临床疗效及治疗前后的视觉模拟评分法(VAS)评分、日本骨科协会评估治疗(JOA)评分。结果对照组总有效率为85.9%(67/78),治疗组为94.9%(74/78),2组比较差异有统计学意义(P<0.05);治疗后2组VAS评分均明显降低,JOA评分均明显升高,与同组治疗前比较差异有统计学意义(P<0.05),且治疗组对各项指标的改善作用明显优于对照组,2组治疗后比较差异均有统计学意义(P<0.05)。结论郭氏仰卧拔伸牵引手法治疗神经根型颈椎病疗效显著,能明显改善患者的临床症状,促进患者整体功能的恢复,值得临床推广应用。Objective To observe the clinical efficacy of cervical radiculopathy( CR) treated by GUO’s pulling traction manipulation in supine position. Methods Totally 156 cases of CR were divided into treatment group and control group according to random number table method,with 78 cases in each.The control group was given routine manipulation and traction,while the treatment group was given GUO’s pulling traction manipulation in supine position. The two groups were both given treatment once every other day. After treatment for 7 times,the clinical efficacy,visual analogue scale( VAS) score and Japanese Orthopedic Association( JOA) score of the two groups before and after treatment were compared. Results The total effective rate of the control group was 85.9%( 67/78),that of the treatment group was 94.9%( 74/78),and the difference was statistically significant between the 2 groups( P<0.05). After treatment,VAS score of the 2 groups decreased significantly,while JOA score increased significantly,and the difference was statistically significant compared with the same group before treatment( P<0.05). The improvement effect of the treatment group on various indicators was significantly better than that of the control group,and the difference was statistically significant between the two groups after treatment( P<0.05).Conclusion It has marked efficacy to use GUO’s pulling traction manipulation in supine position in the treatment of CR,which can significantly improve the clinical symptoms and promote the recovery of the overall function of patients. It deserves clinical promotion.
