临床上应用替考拉宁的合理性分析

摘　　要：目的调查并研究苏州大学附属第三医院临床上使用替考拉宁的情况,为临床更加合理高效使用替考拉宁提供一定的参考依据。方法选取2016年9月至2019年9月苏州大学附属第三医院住院期间使用替考拉宁治疗各种感染的患者165例,使用电子病历系统提取这些患者的临床资料,进一步对其进行统计学分析。结果共有165例患者符合条件使用替考拉宁来治疗各种不同感染,病原学标本送检率达97.0%,送检样本阳性率为54.6%,以肺部感染为主,占62.4%。根据药敏试验和细菌培养的结果使用替考拉宁的患者共47例(28.5%)。共有73例(44.2%)患者使用替考拉宁存在着不合理之处,其中初次使用替考拉宁的负荷剂量给药方案不合理者12例(16.4%),未根据患者的肾功能水平及时调整替考拉宁的使用剂量者16例(21.9%),以及替考拉宁未予起始负荷量者66例(90.4%)等。结论在临床使用替考拉宁的过程中我们应严格规范其使用适应症,给予个体化的用药方案,尤其是在给药负荷剂量及肾功能不全患者使用的剂量调整方面,以便临床上进一步提高替考拉宁的合理应用水平。Objective To investigate and study the rational use of teicoplanin in The Third Affiliated Hospital of Suzhou University, and to provide a reference for more rational and efficient use of teicoplanin. Methods From september 2016 to september 2019, 165 patients with various infections were treated with teicoplanin in The Third Affiliated Hospital of Soochow University. The clinicopathological data of these patients were extracted by electronic medical record system and analyzed statistically. Results A total of 165 patients were eligible to use teicoplanin to treat various infections. The rate of etiological specimens was 97.0%. The positive rate of samples was 54.6%. Pulmonary infections accounted for 62.4%. According to the results of drug susceptibility test and bacterial culture, we selected 47 patients(28.5%) who used teicoplanin. Among them, 73(44.2%) patients had unreasonable use of teicoplanin, mainly manifested by unreasonable loading dose regimen(16.4%) of the initial teicoplanin use, failure to adjust the dose of teicoplanin according to the renal function level of the patients(21.9%) and no initial loading of teicoplanin(90.4%). Wait. Conclusion We should strictly regulate the indications and individual dosage regimens of teicoplanin in the course of clinical use, especially in the dosage regimens of loading dose and patients with renal insufficiency.
