CT对胸部体模内病灶的检出率:不同扫描条件的对比研究

摘　　要：目的:基于胸部X线体模,探讨不同CT扫描条件对病灶检出率的影响。方法:在胸部体模内分别置入三种物质(糯米80粒、绿豆80例和黄豆40粒)模拟不同性质的病灶,每种物质分别采用3种管电压(80、100和120 kV)和5种管电流(10~50 mA)进行CT扫描,共采集45组图像,针对不同管电压和管电流的影像诊断结果分别进行数据统计,采用χ^2检验进行组间比较。结果:随着管电压的降低,对体模内病灶的检出率呈下降趋势,分别为91.9%、89.3%和88.5%,100 kV和80 kV条件下的病灶检出率与120 kV间的差异具有统计学意义(P<0.05)。体模内病灶检出率随管电流的减小呈下降趋势,分别为95.2%(50 mA)、93.3%(40 mA)、92.7%(30 mA)、88.0%(20 mA)和80.3%(10 mA),40 mA和30 mA的病灶检出率与50 mA间的差异无统计学意义(P>0.05),而20 mA和10 mA的病灶检出率与50 mA间的差异有统计学意义(P<0.01)。结论:管电压和管电流是影响CT对胸部体模内病灶检出率的重要因素,临床上应结合病灶类型选择合理的CT扫描参数。Objective:The aim of this study was to investigate the influences of chest X-ray phantom CT scan under different parameters on the detection rate of lesions.Methods:Three types of materials(80 glutinous rice,80 mung beans and 40 soybeans)were implanted into the chest phantom to simulate different lesions.A total of 45 sequence images were collected according to different para-meters of three tube voltages(80,100 and 120kV)and 5 tube currents(10~50mA).Chi-square test was used to analyze the diagnosis at different tube voltages and currents.Results:The lesion detection rates decreased with the decrease of tube voltage,91.9%at 120kV,89.3%at 100kV and 88.5%at 80kV,respectively.There was statistically significant difference in the lesion detection rate of 100kV and 80kV compared with that of 120kV(both P<0.05).As the reduction of tube current,the detection rate of lesions was also decreased,they were 95.2%(50mA),93.3%(40mA),92.7%(30mA),88.0%(20mA)and 80.3%(10mA),respectively.The difference of detection rate of 40mA and 30mA was not statistically significant compared with that of 50mA(P>0.01).However,the difference of the detection rate between 20mA and 50mA,or between 10mA and 50mA was significant(P<0.01).Conclusion:Tube voltage and tube current are the important factors influencing the detection rate of lesions in chest phantom.Reasonable CT scan parameters should be selected according to the type of lesions in lungs.
