摘　　要：随着人们生活水平的提高,人们对各方面尤其是医疗方面的期望值也有所增加,对于自身的健康情况加以重视.传统的社区门诊已不能够全面满足当前人们对健康的需要,而且我国人口老龄化问题已经凸显出来,而老年人也早已与慢性病联系在一起,在种种形势下,有必要探索新的社区卫生管理模式.近年来,"医防融合"模式在社区卫生管理方面得到广泛关注,该模式以提高居民健康水平为宗旨,从家庭医生签约服务入手,发挥基层医疗机构的卫生服务职能.这一模式通过医生与居民签约,进行一对一服务,医生为居民打造个性化的健康干预方案,保障服务工作个性化发展;并且还定期为社区居民举办健康讲座等活动,广泛提高居民的健康认知,整体与个体相结合,广度与深度双向发展,为居民全方位的进行健康管理服务,在社区卫生管理方面起到了显著效果.With the improvement of people's living standards,people's expectations for various aspects,especially medical care,have also increased,and their own health conditions are valued.Traditional community clinics can no longer fully meet the current needs of people for health,and the problem of aging of our population has become prominent,and the elderly have long been associated with chronic diseases.Under various circumstances,it is necessary to explore new community health management mode.In recent years,the"medical and preventive integration"model has received widespread attention in community health management.This model aims at improving the health of residents,starting with family doctors signing services,and playing the health service function of primary medical institutions.In this model,doctors sign a contract with residents to provide one-on-one services.Doctors create personalized health intervention programs for residents to ensure the personalized development of service work.They also regularly organize health lectures and other activities for community residents to improve the health cognition of residents,the integration of the whole and the individual,the breadth and depth of the two-way development,provide comprehensive health management services for residents,and have played a significant role in community health management.
