鼻腔内窥镜下射频消融在急症鼻出血治疗中的应用效果分析

摘　　要：目的研究急症鼻出血在鼻腔内窥镜下应用射频消融的临床治疗效果。方法方便选取该院自2017年6月-2019年6月之间的鼻内镜检查见出血点的急诊鼻出血患者102例作为研究对象,等分为两组,均进行鼻腔内窥镜下射频消融治疗,观察组术后进行膨胀海绵填塞,对照组不填塞。观察患者一次治愈率和术后并发症发生率等情况,分析临床治疗效果。结果从一次治愈率看,观察组治愈的患者数50例(98.0%)高于对照组的46例(90.2%),差异有统计学意义(χ^2=8.641,P<0.05);观察组平均住院时间(3.3±0.15)d,优于对照组(4.2±0.63)d,差异有统计学意义(t=9.822,P<0.05)。观察组并发症发生率3.9%,低于对照组的9.8%,差异有统计学意义(χ^2=11.637,P<0.05)。结论应用鼻腔内镜下射频消融法治疗急症鼻出血具有良好的治疗效果,与此同时填塞适量膨胀海绵能显著提升了一次治愈率,降低了患者术后鼻腔并发症,治疗安全性较高,在缩短住院时间的同时,节省了医院的医疗资源、缩短了患者的康复时间、减轻患者的痛苦和经济负担。Objective To study the clinical effect of radiofrequency ablation under nasal endoscope for acute nosebleeds.Methods 102 patients with emergency nosebleeds whose bleeding points were seen by nasal endoscopy between June 2017 and June 2019 were conveniently selected as the research subjects.They were equally divided into two groups,all underwent endoscopic RF ablation treatment,the expansion group was packed with expansion sponge after operation,and the control group was not packed.The primary cure rate and postoperative complication rate were observed,and the clinical treatment effect was analyzed.Results From the perspective of the primary cure rate,the number of patients cured in the observation group was 50(98.0%)higher than that in the control group(46)(90.2%),the difference was statistically significant(χ^2=8.641,P<0.05);the average hospital stay in the observation group(3.3±0.15)d,better than the control group(4.2±0.63)d,he difference was statistically significant(t=9.822,P<0.05).The incidence of complications in the observation group was 3.9%,which was lower than the control group's 9.8%,he difference was statistically significant(χ^2=11.637,P<0.05).Conclusion The application of radiofrequency ablation under nasal cavity endoscopy has a good therapeutic effect in treating acute nosebleeds.At the same time,filling an appropriate amount of expanding sponge can significantly improve the primary cure rate,reduce postoperative nasal complications,and have higher safety.While shortening the length of hospital stay,it saves the hospital's medical resources,shortens the patient's recovery time,relieves the patient's pain and economic burden.
