“互联网+延续护理”干预模式对中青年脑出血出院居家康复患者独立生活能力的影响

摘　　要：目的探讨"互联网+延续护理"干预模式对中青年脑出血出院居家康复患者独立生活能力的影响。方法选取2015年01月~2019年12月我院收治的46例15~50岁成人中青年高血压脑出血术后患者,患者住院期间治疗和护理方案相同,按照出院后延续护理干预模式不同随机分为对照组(n=22)和观察组(n=24)。对照组接受住院教育联合电话回访和现场指导的常规延续护理模式,观察组于出院前通过微信扫码加入居家康复健康管理群,接受"互联网+延续护理"干预模式。比较两组出院时和干预3个月的独立生活能力各项指标及总分的差异水平。结果出院时,两组患者独立生活能力总分以及行动能力、社会认知能力、交流能力、转移能力和自我照顾能力各指标比较均无明显差异,差异无统计学意义(P>0.05);干预3个月后,两组各指标比较差异明显,观察组各指标评分均明显高于对照组,差异有统计学意义(P<0.05)。结论应用"互联网+延续护理"干预模式,能显著促进中青年脑出血出院居家康复患者独立生活能力的恢复,患者和家属易于接受,可广泛推广应用。Objective To explore the influence of"internet+continuous nursing"intervention mode on the independent living ability of middle-aged and young patients with cerebral hemorrhage discharged from home.Methods A total of 4615-to 50-year-old adults with hypertensive cerebral hemorrhage who were admitted to our hospital from January 2015 to December 2019 were selected.The treatment and nursing plan were the same during hospitalization,and they were randomly divided according to different nursing intervention models after discharge.Control group(n=22)and observation group(n=24).The control group received the routine continuous nursing mode of in-patient education combined with telephone return visits and on-site guidance.The observation group joined the home rehabilitation health management group via WeChat scan code before leaving the hospital,and accepted the"Internet+extended nursing"intervention mode.The differences in the indicators of independent living ability and the total score at the time of discharge and 3 months of intervention were compared between the two groups.Results At the time of discharge,there was no significant difference in the total scores of independent living ability and the indicators of mobility,social cognitive ability,communication ability,transfer ability,and self-care ability between the two groups(P>0.05);after 3 months of intervention,The comparison of each index was significantly different,and the scores of each index in the observation group were significantly higher than those in the control group(P<0.05).Conclusion The application of the"Internet+continuous nursing"intervention model can significantly promote the recovery of the independent living ability of young and middle-aged patients with cerebral hemorrhage discharged from home,which is easy for patients and their families to accept,and can be widely promoted and applied.
