摘　　要：目的分析北京市丰台区严重异常反应病例发生特征,评价预防接种安全性,探讨疫苗潜在和罕见的严重异常反应。方法通过中国免疫规划信息管理系统疑似预防接种异常反应(adverse event following immunization,AEFI)信息管理系统,收集2012-2018年丰台区严重异常反应病例,进行描述性流行病学分析。结果2012-2018年丰台区AEFI共722例,报告发生率13.78/10万剂;其中严重异常反应11例,报告发生率0.21/10万剂;男性高于女性,年龄多在2岁以内,共涉及9种疫苗。6(6/11)例反应在接种第1剂次疫苗后发生;11例严重异常反应病例中,4例血小板减少性紫癜,4例热性惊厥,2例抽搐,1例过敏性休克。结论丰台区AEFI监测敏感性较高,严重异常反应发生率远低于预期,疫苗安全性和预防接种服务质量良好。Objective To determine characteristics of serious rare adverse reactions(SRAR)in Fengtai district,and to evaluate the safety of vaccination and explore the potential serious rare adverse reactions.Methods Cases of serious rare adverse reaction reported through the National Immunization Management Systemin in Fengtai district from 2012-2018 were collected and analyzed by descriptive epidemiological method.Results A total of 722 AEFI cases were reported,and the incidence rate was 13.78 per 100000 doses.Among them,11(0.21 per 100000 doses)were serious rare adverse reaction cases.The serious rare adverse reaction incidence in males was higher than that in females.Nine kinds of vaccine were involved.Most cases were under 2 years old.Up to 6 SRAR cases occurred following the first dose of vaccination.SRAR cases were thrombocytopenia purpura(4 cases),febrile convulsion(4 cases),convulsion(2 cases)and anaphylaxis(1 case).Conclusions AEFI surveillance maintained a high sensitivity in Fengtai district.The serious rare adverse reaction incidence was much lower than the expected.Both the safety of vaccines and the quality of immunization services in Fengtai district were good.
