血清淀粉酶样蛋白A对诊断新生儿败血症的准确性的Meta分析

摘　　要：目的系统评价血清淀粉酶样蛋白A(SAA)对诊断新生儿败血症(NS)的准确性。方法全面检索Pubmed、Embase、Cochrane Library、万方、中国知网等相关文献数据库,检索时间均为建库至2019年7月18日,检索语言不限。依据纳入标准筛选文献,采用QUADAS-2工具对纳入文献进行质量评价,使用Review Manager 5.2、Metadisc 1.4和Stata15.0软件进行Meta分析。结果共纳入14篇研究,1146例新生儿,SAA诊断NS合并敏感性为0.87(95%CI为76%~94%),合并特异性为0.88(95%CI=80%~93%),DOR值为51.30(95%CI=16.90~155.72),SROC曲线AUC为0.94(95%CI=0.91~0.96)。结论SAA诊断NS有较高的灵敏度及特异度,可用于NS的诊断。Objective To systematically evaluate the accuracy of serum amyloid-like protein A(SAA)in the diagnosis of neonatal sepsis(NS).Methods A comprehensive search of Pubmed,Embase,Cochrane Library,Wanfang,China Knowledge Network and other related literature databases was conducted.The search time was from July 18,2019,and the search language was not limited.The literature was selected according to the inclusion criteria,and the quality of the included literature was evaluated using the QUADAS-2 tool.The meta-analysis was performed using Review Manager 5.2,Metadisc 1.4 and Stata 15.0 software.Results A total of 14 articles and 1146 newborns were included in the study.The sensitivity of SAA diagnosis of NS was 0.87(95%CI:76%-94%),and the combined specificity was 0.88(95%CI:80%-93%).The DOR value was 51.30(95%CI:16.90-155.72),and the SROC curve AUC was 0.94(95%CI:0.91-0.96).Conclusion SAA has a high sensitivity and specificity for the diagnosis of NS and can be used for the diagnosis of NS.
