摘　　要：目的探讨URBP、UHP、UPCX和UNephrin联合检测对糖尿病肾病早期诊断的价值。方法选择2型糖尿病患者180例(分3组),对照组60例。比较4组URBP、UHP、UPCX和UNephrin水平,分析各指标和联合检测预测DN的ROC及AUC。为减少受试者尿液浓度变化对测试结果的影响,结果均以除尿肌酐(UCR)的相应结果表示。结果UPCX/UCR水平DM组与对照组差异无统计学意义(P>0.05);其他各组间两两比较差异均有统计学意义(P<0.05)。UHP:AUC为0.772,特异度为56.91%,敏感度为85.47%;UPCX:AUC为0.759,特异度为51.22%,敏感度为88.89%;UNephrin:AUC为0.777,特异度为64.23%,敏感度为78.63%;URBP:AUC为0.890,特异度为85.37%,敏感度为78.63%;联合检测:AUC为0.924,特异度为86.18%,敏感度为88.89%。联合检测和单独检测相比,敏感度和特异度明显增高,差异有统计学意义(P<0.05)。结论URBP、UHP、UPCX和UNephrin联合检测在早期糖尿病肾病诊断中具有重要的临床价值。Objective To evaluate the value of combined detection of URBP,UHP,UPCX and UNephrin in early diagnosis of diabetic nephropathy.Methods A total of 180 patients with type 2 diabetes were divided into three groups and there are 60 normal patients in control group(N).The levels of RBP,HP,PCX and Nephrin in urine were compared among the four groups.The ROC curve and AUC of each index and combined detection index were analyzed to predict DN.In order to reduce the effect of urine concentration change on the test results,the results were expressed as the corresponding results of urine creatinine(UCR).Results There was no statistical significance on the difference in UPCX/UCR level between DM group and the control group(P>0.05),but there was statistical significance on the difference between other groups(P<0.05).UHP:AUC was 0.772,specificity 56.91%,and sensitivity 85.47%.UPCX:AUC was 0.759,specificity 51.22%and sensitivity 88.89%.UNephrin:AUC was 0.777,specificity was 64.23%and sensitivity was 78.63%.URBP:AUC was 0.890,specificity 85.37%and sensitivity 78.63%.Combined detection:AUC was 0.924,specificity was 86.18%,and sensitivity was 88.89%.The sensitivity and specificity of the combined detection are higher than that of single detection,with the differ-ences statistically significant(P<0.05).Conclusion The combined detection of URBP,UHP,UPCX and UNephrin has important clinical value in the diagnosis of early diabetic nephropathy.
