摘　　要：目的对市售2种国产、进口筛选细胞、谱细胞的不规则抗体检测性能进行评估,并讨论目前抗体筛选及鉴定的现状。方法从血型系统和抗原数目等多项指标进行评价。根据抗体鉴定概率的原则从理论上对国内报道过的稀有血型抗体进行漏检分析。使用标准化的抗血清对几种抗体作抗体筛选体外模拟试验并进行漏检分析。结果国产1号细胞漏检抗-E、-e、-Fyb、-Lea等稀有抗体。2号国产细胞漏检抗-Fya,且Fyb、M、N、S阳性细胞存在过多杂合子抗原。国产细胞可检出大部分常见抗体和一部分稀有抗体,如:Rh、MN、P、Lewis等。进口2号谱细胞漏检抗-e抗体。2种进口品牌抗体鉴定能力以下几种抗原优势较为明显:E、e、K、Leb、Fya、Fyb、Jka。结论进口细胞在血型系统和抗原的数量及设置、不规则抗体检测能力上要优于国产细胞。我国的筛选细胞应根据种族血型特点制定适合自己的规范,在不规则抗体检测和新生儿溶血病的预防中发挥更大的作用。Objective To evaluate the detection performance of two domestic and imported screening cells and spectrum cells for irregular antibodies,and to discuss the current status of antibody screening and identification.Methods Evaluation is made from multiple indicators such as the blood group system and the number of antigens.According to the principle of antibody identification probability,the rare blood group antibodies reported in China were theoretically missed.Using standardized antisera,several antibodies were screened for in vitro mock tests and missed analysis.Results The domestic No.1 cells missed detection of rare antibodies such as anti-E,-e,-Fyb,and-Lea.No.2 domestic cells missed detection of anti-Fya,and Fyb,M,N,S positive cells had too many heterozygous antigens.Domestic cells can detect most common antibodies and some rare antibodies,such as Rh,MN,P,Lewis,etc.Imported No.2 spectrum cell missed anti-e antibody detection.The identification capabilities of the two imported brand antibodies are obvious in the following antigens:E,e,K,Leb,Fya,Fyb,Jka.Conclusion Imported cells are superior to domestic cells in the number and settings of blood group systems and antigens,and the ability to detect irregular antibodies.The screening cells in our country should formulate their own specifica-tions according to the characteristics of our blood type,and play a greater role in the detection of irregular antibodies and the prevention of neonatal hemolytic disease.
