摘 要:目的探讨脓毒血症患者外周血单个核细胞Toll样受体和mi RNA23表达与患者临床特点的关系。方法80例脓毒血症患者为本院2016年1日-2018年11月在本院就诊的患者,提取外周血单个核细胞,检测外周血Toll4样受体m RNA和mi RNA23表达,检测血清中TNF-a、IL-6、IL-8水平,对比不同水平Toll样受体和mi RNA23表达水平患者临床特点及炎症指标,Logistics回归分析探讨患者临床特点与脓毒血症预后的关系。结果Toll4 m RNA和mi RNA23低水平患者心率、SOFA评分、APACHII评分水平明显低于对应高水平的患者,TNF-a、IL-6、IL-8水平明显高于对应低水平的患者,氧分压明显高于对应高水平的患者,上述差异均有统计学意义(P<0.05),Toll m RNA、mi RNA 23、TNF-a、IL-6、IL-8、APACHE II评分、SOFA评分是患者死亡的独立危险因素(P均<0.05)。结论外周血单个核细胞Toll m RNA和mi RNA23表达与患者病情及预后密切相关。Objective To investigate the relationship between the expressions of Toll-like receptor and microRNA23 in periph-eral blood mononuclear cells of patients with sepsis and their clinical characteristics.Methods Eighty patients with sepsis in our hospital from January 2016 to November 2018 were studied.Peripheral blood mononuclear cells were extracted,Toll-4-like receptor(Toll-4-like receptor)mRNA and microRNA23 expression in peripheral blood were detected,TNF-a,IL-6 and IL-8 levels in serum were detected,and the clinical characteristics and inflammation indexes of patients with different levels of Toll-like receptor and microRNA23 expression were compared.Logistic regression analysis was performed to analyze the relationship between clinical characteristics and prognosis of sepsis.Results The heart rate,SOFA score and APACHII score of patients with low Toll 4 mRNA and microRNA 23 levels were significantly lower than those of patients with correspond-ing high levels.The levels of TNF-a,IL-6 and IL-8 were significantly higher than those of patients with low levels,and the partial pressures were higher than the patients with high level of oxygen,with the above differences statistically significant(P<0.05).Toll mRNA,microRNA23,TNF-a,IL-6,IL-8,APACHE II,SOFA were independent risk factors for death(P<0.05).Conclusion The expressions of Toll mRNA and microRNA23 in peripheral blood mononuclear cells are closely related to patients'condition and prognosis.
