肾功能损害相关指标、生长因子及Th1/Th2细胞因子相关水平与妊娠期高血压综合征的相关性分析

摘　　要：目的探讨肾功能损害相关指标、生长因子及Th1/Th2细胞因子与HDPC的关系。方法选取妊娠期高血压综合征(HDPC)患者77例,按照患病的严重程度分为妊娠期高血压组30例,轻度子痫前期组25例,重度子痫前期组27例。选择同时期孕期健康体检的妇女35例作为对照组,检测4组的肾功能损害相关指标Cys-C、β2-MG、Cr、VEGF、s Flt-1、IGF-1及Th1/Th2细胞因子相关水平IL-2、INF-γ、TNF-α、IL-4、IL-10和IL-13与HDPC的相关性。结果HDPC患者肾功能损伤相关指标Cys-C、β2-MG水平明显高于对照组,并且随着病情的加重Cys-C、β2-MG水平呈递增趋势;HDPC患者VEGF、IGF-1的表达明显低于对照组,并且病情越严重,这2种细胞因子水平越低,s Flt-1生长因子水平明显高于对照组,并且随着病情严重递增;HDPC患者Th1类细胞因子INF-1、TNF-α和Th2类细胞因子IL-4、IL-10、IL-13水平明显高于对照组,且病情越严重细胞因子水平越高;与对照组相比,HDPC患者Th1类细胞因子IL-2水平明显降低,并且随着病情加重,降低越明显。结论联合检测肾功能相关指标、生长因子及Th1/Th2细胞因子妊娠期高血压综合征相关水平对判断HDPC患者病情的严重程度具有重要的临床意义。Objective To investigate the relationship between renal dysfunction-related indicators,growth factors and Th1/Th2 cytokines and hypertensive disorder complicating pregnancy(HDCP).Methods Seventy-seven patients with HDCP were enrolled.According to the severity of the disease,all the subjects were divided into gestational hypertension group(30 patients),mild preeclampsia group(25 patients),and severe preeclampsia group(27 patients).Thirty-five women who underwent healthy physical examination during the same period of pregnancy were selected as the control group.The renal dysfunction related indicators Cys-C,β2-MG,Cr,growth factor VEGF,sFlt-1,IGF-1 and Th1/Th2 cytokines of the four groups were detected to find their correlation with HDCP.Results The levels of Cys-C andβ2-MG in HDCP patients were significantly higher than those in the control group,and the levels of Cys-C andβ2-MG increased with the severity of the disease.The expressions of VEGF and IGF-1 in HDCP patients is significantly lower than the control group,and the more serious the disease,the lower the level of these two cytokines.The level of sFlt-1 growth factor was significantly higher than the control group,and with the severity of the disease.The Th1 cytokine INF-1,TNF-αin HDPC patients and Th2 cytokines IL-4,IL-10,IL-13 levels were significantly higher than the control group,and the heavier the disease,the higher the cytokine level.Compared with the control group,the level of Th1 cytokine IL-2 in HDPC patients is significantly reduced,and as the condition worsens,he decrease is more obvious.Conclusion Combined detection of renal function related indicators,growth factors and Th1/Th2 cytokine related levels have clinical significance in judging the severity of HDPC patients.
