摘  要:目的:比较不同电针频率对神经病理痛维持期的镇痛效应,并探讨其脊髓背角(SCDH)中瞬时受体电位香草酸亚型1(TRPV1)、P2X3干预机制。方法:大鼠完全随机分为坐骨神经分支选择性损伤(SNI)模型组、假模型组、2Hz电针组、100Hz电针组和2/100Hz电针组。除假模型组外,其余各组进行SNI手术建立神经病理痛模型。取双侧足三里、昆仑进行电针干预。采用Von Frey丝检测各组大鼠各时间点的患侧足底机械缩足阈(PWTs);采用免疫印迹法测量SNI维持期(造模后7d)大鼠患侧L4-L6 SCDH TRPV1、p-TRPV1、P2X3的表达。结果:与SNI模型组比较,各电针组均能升高SNI大鼠患侧PWTs(P<0.05),其中,治疗1d后2Hz电针的提升效果最显著;与假模型组比较,SNI维持期大鼠L4-L6 SCDH TRPV1、p-TRPV1、P2X3蛋白表达显著升高(P<0.05),而各电针组均能有效降低上述蛋白的过表达(P<0.05),其中,2Hz电针对SNI维持期大鼠SCDH p-TRPV1蛋白过表达的下调作用最为显著(P<0.05)。结论:2Hz、100Hz和2/100Hz电针均能有效缓解SNI维持期大鼠的神经病理痛,该镇痛效应可能与其有效抑制SCDH TRPV1、p-TRPV1和P2X3蛋白表达有关;2Hz对SNI维持期大鼠的优势镇痛效应可能与其强效抑制SCDH p-TRPV1的过表达有关。Objective: To compare the analgesic effects of different electroacupuncture frequencies on the maintenance of neuropathic pain, and to explore the mechanism of TRPV1 and P2 X3 intervention in SCDH. Methods: SD rats were randomly divided into SNI model group, sham model group, 2 Hz EA group, 100 Hz EA group and 2/100 Hz EA group. In addition to the sham model group, the other groups underwent SNI surgery to establish neuropathic pain model. EA was administered at the acupuncture-points of bilateral Zusanli(ST36) and Kunlun(BL60). The PWTs were detected by Von Frey hairs and observed at different time points after EA stimulation in each group. Western blot was used to test the expressions of TRPV1, p-TRPV1 and P2 X3 in ipsilateral SCDH of SNI maintenance rats(7 d after modeling). Results: Compared with SNI model group, the 2 Hz, 100 Hz and 2/100 Hz EA could increase the ipsilateral PWTs of the SNI rats(P<0.05), among which 2 Hz EA had the most remarkable analgesia effect after treatment for one day. Compared with sham model group, the protein expressions of SCDH TRPV1, p-TRPV1 and P2 X3 in SNI maintenance rats were significantly increased(P<0.05);2 Hz, 100 Hz and 2/100 Hz EA could reduce the overexpression of SCDH TRPV1, p-TRPV1 and P2 X3 in SNI maintenance rats(P<0.05), while 2 Hz EA had the most significant down-regulation effect on the overexpression of SCDH p-TRPV1 in SNI maintenance rats(P<0.05). Conclusion: 2 Hz, 100 Hz and 2/100 Hz EA can effectively relieve SNI-induced neuropathic pain, and its analgesic effect may be related to the intervention mechanisms of SCDH TRPV1, p-TRPV1 and P2 X3 of SNI maintenance rats. 2 Hz is the optical frequency of EA intervention of neuropathic pain, and its dominant analgesic effect may be realized by strongly inhibiting the overexpression of SCDH p-TRPV1 of SNI maintenance rats.
