乳腺癌前哨淋巴结二维及三维超声造影的对比研究

摘　　要：目的探讨二维及三维超声造影在乳腺癌前哨淋巴结活检中的临床价值。方法在我院行前哨淋巴结二维超声造影(2D-CEUS)及三维超声造影(3D-CEUS)检查并手术证实的早期乳腺癌患者68例,随机分为两组,2D-CEUS组和3D-CEUS组,观察前哨淋巴结的数目、大小、部位、淋巴管引流模式及增强模式,并记录两组超声造影检查时间以及前哨淋巴结活检完成时间。结果以前哨淋巴结活检术中蓝染及病理结果作为金标准,2D-CEUS组与3D-CEUS组检查的引流淋巴管定位准确率及前哨淋巴结超声造影准确率差异无统计学意义(P>0.05),2D-CEUS组与3D-CEUS组平均检查所需时间差异无统计学意义(P>0.05),手术时间差异有统计学意义(P<0.05)。结论乳腺前哨淋巴3D-CEUS在2D-CEUS基础上,能够三维立体形象显示引流淋巴管及前哨淋巴结,有利于提高临床医师对前哨淋巴结的深度及空间分布的认知,减少术中探查时间,值得临床推广应用。Objective To compare the clinical value of two-dimensional and three-dimensional contrast-enhanced ultrasound in sentinel lymph node biopsy of patients with breast cancer. Methods 68 patients with early breast cancer confirmed by surgery were included in our study. They were randomly divided into two groups: 2 D-CEUS group and 3 D-CEUS group. Two-dimensional contrast-enhanced ultrasound(2 D-CEUS) and three-dimensional contrast-enhanced ultrasound(3 D-CEUS) were performed on patients in the two groups respectively.The number, size, location, lymphatic drainage pattern and enhancement pattern of sentinel lymph nodes in all patients included were observed and recorded. The contrast-enhanced ultrasound examination time and completion time of sentinel lymph node biopsy in the two groups were recorded. Results Blue staining during the sentinel lymph node biopsy and pathological results were used as the gold standard. There was no significant difference in the accuracy of lymphatic vessel positioning in the 2 D-CEUS group and the 3 D-CEUS group(P>0.05). And no significant difference was found in diagnostic accuracy of sentinel lymph node metastasis in the 2 D-CEUS group and the 3 D-CEUS group(P>0.05). Otherwise, there was no significant difference in the average examination time between the 2 D-CEUS group and the 3 D-CEUS group(P>0.05), and the difference in the operation time between these two groups was significant(P<0.05). Conclusions Based on 2 D-CEUS,3 D-CEUS of breast sentinel lymph nodes can display three-dimensional images of lymph vessels and sentinel lymph nodes, which is conducive to improving clinicians’ awareness of the depth and spatial distribution of sentinel lymph nodes. And it’s good for reducing the time of intraoperative exploration and worth clinical application.
