乳腺实性乳头状癌的多模态超声表现

摘　　要：目的探讨乳腺实性乳头状癌的多模态超声表现。方法回顾性分析51例经病理证实的乳腺实性乳头状癌的灰阶超声、彩色多普勒超声及弹性成像表现,其中18例行超声造影并观察其造影表现。结果乳腺实性乳头状癌主要超声形态有肿块型、腺体型和导管型。其主要的超声表现为伴或不伴液性区的实性肿块、平行位、边缘不光整、后方回声增强或无改变,可伴导管扩张,内部血供较丰富,弹性成像较硬。造影表现为中高强度灌注,造影模式以快进快退为主,造影后肿块大小多数有增大。结论乳腺实性乳头状癌的超声影像学表现具有一定的特征,综合多种超声检查手段,有利于对其进行正确的诊断和评估。Objective To investigate the multimodal ultrasonographic manifestations of solid papillary carcinoma(SPC) of the breast. Methods Gray-scale ultrasonography, color doppler ultrasonography and elastosonography were retrospectively analyzed in 51 cases of pathologically confirmed SPC, and 18 cases of them were examined and assessed by contrast-enhanced ultrasound(CEUS). Results There are three main ultrasonographic forms of SPC: mass type, glandular type and ductal type. The main ultrasonographic manifestations of SPC were solid masses with or without fluid areas, parallel position, irregular margins, enhanced or unchanged posterior echo, catheter dilatation of some cases, abundant internal blood supply, and hard elastography. The CEUS performance of SPC was characterized by medium and high perfusion, fast flow-in and fast wash-out, and increased in size of majority cases. Conclusions The ultrasonographic manifestations of SPC of breast have certain characteristics. The combination of various ultrasonographic methods is conductive to the correct diagnosis and evaluation of SPC of the breast.
