摘 要:目的研究肝细胞性肝癌(HCC)患者超声检测血流分级与血清HMGB1、SCCAg、P53蛋白表达的关系,分析疾病进展程度。方法选择2017年2月至2019年2月在绵阳市中心医院住院的HCC患者120例,其中男性59例,女性61例;年龄42~65岁,平均年龄53.48岁;体质量指数(BMI)20.30~27.82 kg/m^2,平均BMI为24.06 kg/m^2;单一病灶69例,多发病灶51例;肿瘤直径1.5~11.4 cm,平均直径为4.22 cm。行超声检查和实验室检测血清HMGB1、SCCAg、P53蛋白表达水平。分析病灶大体类型、血流分级的HMGB1、SCCAg、P53蛋白表达水平之间的差异。研究血流分级与HMGB1、SCCAg、P53蛋白表达的相关性。结果超声诊断小肝癌型35例,小结节型39例,巨块型46例。血流分级Ⅰ级42例,Ⅱ级37例,Ⅲ级41例。病灶大体类型、血流分级的HMGB1、SCCAg、P53蛋白表达水平比较,差异存在统计学意义(P<0.05);HMGB1、SCCAg、P53蛋白表达水平从高到低依次为巨块型、结节型、小肝癌型,病灶大体类型、血流分级与血清HMGB1、SCCAg、P53蛋白表达水平呈现正相关。结论HCC患者的HMGB1、SCCAg、P53蛋白表达水平与患者的病灶大体类型、血流分级均呈现显著正相关。Objective To study the relationship between blood flow grading and expression of serum HMGB1, SCCAg, P53 protein in patients with hepatocellular carcinoma(HCC), and analyze the degree of disease progression. Methods From February 2017 to February 2019, a total of 120 HCC patients were enrolled, which included 59 males and 61 females, aged 42-65 years old with mean age of 53.48 years old;body mass index(BMI) was 20.30-27.82 kg/m^2 with mean BMI of 24.06 kg/m^2.There were 69 patients with single lesion and 51 with multiple lesions. The diameter of tumor was 1.5-11.4 cm with mean diameter of 4.22 cm. All patients were given ultrasonography and laboratory tests to detect expression levels of serum HMGB1,SCCAg and P53 protein. The differences in general types of lesions, blood flow grading HMGB1, SCCAg and P53 protein expression levels were analyzed. The relationship between blood flow grading and expression of HMGB1, SCCAg, P53 protein were investigated. Results There were 35 cases of small liver cancer type, 39 of small nodule type and 46 of massive type were found by ultrasonic examination. There were 42 cases of grade Ⅰ, 37 of grade Ⅱ, and 41 of grade Ⅲ by blood flow grading. There were significant differences in expression levels of HMGB1, SCCAg and P53 protein in general types of lesion and blood flow grading(P < 0.05);The expression levels of HMGB1, SCCAg and P53 protein from high to low were massive type,small nodule type and small liver cancer. The general types and blood flow grading were positively correlated with expression level of serum HMGB1, SCCAg and P53 protein. Conclusion It is demonstrated that the expression level of HMGB1, SCCAg and P53 protein in HCC patients are significantly positively correlated with general lesion type and blood flow grading.
