摘  要:目的:探讨CT平扫与动态增强扫描在诊断孤立性肺结节的应用价值。方法:对我院查体检出的35例孤立性肺结节患者分别进行CT平扫和动态增强扫描,其中结节直径≤10mm的11例,11~20mm的13例,21~30mm的11例;有2名高年资医师根据CT平扫、动态增强影像资料对肺结节分别做出诊断,全部病例经肺穿刺、胸腔镜或手术病理确诊。分析两种检查方法在诊断肺结节中的应用价值。结果:35例孤立性肺结节患者CT平扫诊断正确率为62.9%,动态增强扫描为80.0%,二者比较差异无统计学意义(P>0.05)。CT平扫与动态增强对直径≤10mm肺结节诊断正确率分别为36.4%和72.7%,二者比较差异有统计学意义(P<0.05)。对直径11~20mm肺结节诊断正确率分别为69.2%和76.9%,直径21~30mm肺结节诊断正确率分别为81.8%和90.9%,以上二组分别比较差异均无统计学意义(P>0.05)。结论:CT平扫与动态增强扫描是诊断孤立性肺结节的重要方法,动态增强扫描能提高诊断正确率,尤其对直径≤10mm形态特征不明显肺结节的早期诊断价值明显。Objective To explore the value of CT plain scan and dynamic enhanced scan in the diagnosis of solitary pulmonary nodules.Methods 35 patients with solitary pulmonary nodules detected by physical examination in our hospital underwent plain CT scan and dynamic enhanced scan.There were 11 cases with nodule diameter≤10mm,13 cases with 11~20mm,and 11 cases with 21~30mm;Two senior physicians made separate diagnosis of lung nodules based on CT plain scan and dynamic enhanced imaging data.All cases were confirmed by lung puncture,thoracoscopy,or surgical pathology.The value of two examination methods in the diagnosis of pulmonary nodules was analyzed.Results The diagnostic accuracy of CT plain scan in 35 patients with solitary pulmonary nodules was 62.9%,and the dynamic enhanced scan was 80.0%.There was no significant difference between the two(P>0.05).The accuracy of CT plain scan and dynamic enhancement for the diagnosis of pulmonary nodules≤10mm in diameter was 36.4%and 72.7%,respectively,and the difference was statistically significant(P<0.05).The correct diagnosis rates of pulmonary nodules with diameters of 11 to 20 mm were 69.2%and 76.9%,and the correct diagnosis rates of pulmonary nodules with diameters of 21 to 30 mm were 81.8%and 90.9%,respectively.There was no significant difference between the two groups(P>0.05).Conclusion CT plain scan and dynamic enhanced scan are important methods for the diagnosis of solitary pulmonary nodules.Dynamic enhanced scans can improve diagnostic accuracy,In particular,early diagnosis of the morphological characteristic diameter≤10mm obvious significant pulmonary nodules.
