阴道彩超对卵巢黄体破裂和宫外孕破裂的鉴别诊断研究

摘　　要：目的:探讨卵巢黄体破裂和宫外孕破裂应用阴道彩超的鉴别诊断效果。方法:随机选取2019年1月—2020年1月某院妇科诊治的宫外孕破裂(70例)和卵巢黄体破裂(70例)患者参与此次研究调查。两组均进行阴道超声和二维超声检查,分析讨论两组检查结果的差异性。结果:经二维超声检查可知,宫外孕破裂组有7例出现假性妊娠囊,21例患者可见胎芽和原始心管搏动;经阴道超声检查可知,卵巢黄体破裂组多数皆显现(半)环状血流信号,未有点状血流信号显现,而血流频谱以(超)低阻力型为主,未出现无张力期和舒张期血流频谱,宫外孕破裂组主要以点状血流信号为多见,血流频谱主要以高阻力型为多见,无张力期和舒张期的血流频谱为0,其中两组中可见盆腔积液比例更高的一组是卵巢黄体破裂组;阴道超声血流动力学指标中RI、PI较高的一组是宫外孕破裂组,而PSV较高的一组则是卵巢黄体破裂组,两组相关指标具有差异性和统计学意义(P<0.05)。结论:阴道超声相较于二维超声的诊断效果更优,可为宫外孕破裂和卵巢黄体破裂患者提供更加准确的诊断信息和参考依据。Objective To investigate the differential diagnosis of Ovarian Corpus luteum rupture and ectopic pregnancy rupture by transvaginal color doppler ultrasound.Methods A total of 70 cases of ruptured ectopic pregnancy and 70 cases of ruptured Corpus Luteum were randomly selected from January 2019 to January 2020.Vaginal ultrasound and twodimensional ultrasound were performed in both groups,and the differences of the results were analyzed and discussed.Results According to the results of two-dimensional ultrasonography,7 cases of ectopic pregnancy rupture group had pseudocyst,21 cases of ectopic pregnancy rupture group had fetal bud and primitive heart tube pulsation,and most cases of ruptured Corpus luteum showed(semi)circular blood flow signal,no point-like blood flow signals were found,but the blood flow spectrum was mainly(ultra)low resistance type,and there were no tension-free and diastolic blood flow spectrum.In the rupture group of ectopic pregnancy,the blood flow spectrum was mainly(high resistance type),the frequency of blood flow in atonic and diastole periods was 0,and the group with higher proportion of Pelvic effusion was the group with ruptured Corpus Luteum,the group with Higher Ri and Pi in vaginal ultrasound was the group with ruptured ectopic pregnancy,and the group with Higher Ri and Pi in vaginal ultrasound was the group with ruptured Hemodynamics,the group with higher PSV was the group with ruptured Corpus Luteum,and there was significant difference between the two groups(P<0.05).Conclusion Vaginal ultrasound is more effective than two-dimensional ultrasound in the diagnosis of rupture of ectopic pregnancy and rupture of Corpus luteum ovary to provide more accurate diagnostic information and reference.
