三维斑点追踪成像在AMI左心室重构中的运用

摘　　要：目的研究三维斑点追踪成像在,急性心肌梗死(acute myocardial infarction,AMI)左心室重构评估中的临床意义。方法选取2017年12月-2019年1月我院收治的急性心肌梗死患者30例作为研究组,选取同期的35例健康志愿者作为对照组,两组参与者都采用三维斑点追踪成像进行检测。比较两组三维斑点追踪参数[左心室心肌整体面积应变(global area strain of left ventricular myocardium,GAS)、左心室整体径向应变(global radial strain of left ventricle,GRS)、左心室整体纵向应变(global longitudinal strain,GLS)、左心室重构血清指标Ⅲ型N端胶原前肽(typeⅢN-terminal procollagen,NⅢNP)、成纤维细胞生长因子23 (fibroblast growth factor 23,FGF23)、C端胶原前肽(C-terminal procollagen,PICP)],通过Person相关分析法分析上述指标与参数的相关性。结果研究组的GAS、GLS、GRS均显著低于对照组(P<0.05);PICP、PⅢNP、FGF23均显著高对照组(P <0.05);研究组的GAS、GLS、GRS均与PICP、PⅢNP、FGF23水平具呈负相关(P<0.05)。结论对AMI左心室重构中使用三维斑点追踪成像,能有效准确的对早期病情进行评估,有利于后期的治疗。Objective To study the clinical significance of three-dimensional speckle tracking imaging in the assessment of left ventricular remodeling in AMI.Methods 30 patients with acute myocardial infarction admitted to our hospital from December 2017 to January 2019 were selected as the study group,and 35 healthy volunteers were selected as the control group.Participants in both groups were detected using three-dimensional spot tracking imaging,two groups of 3 d spot tracking parameters were compared(GAS,GRS,GLS,NⅢNP,FGF23,PICP),the Person correlation analysis method was used to analyze the correlation between the above indicators and parameters.Results GAS,GLS and GRS in the study group were significantly lower than those in the control group(P <0.05).PICP,PⅢNP and FGF23 were significantly higher in the control group(P <0.05).GAS,GLS and GRS in the study group were negatively correlated with the levels of PICP,PⅢNP and FGF23(P <0.05).Conclusion Using threedimensional speckle tracking imaging in AMI left ventricular reconstruction can effectively and accurately evaluate the early stage of disease,and good for later treatment.
