彩超联合经颅多普勒诊断颈部血管狭窄研究

摘　　要：目的分析彩色多普勒超声联合经颅多普勒超声诊断颈部血管狭窄的临床效果。方法选取2019年1月—12月我院收治的76例疑似颈部血管狭窄患者为研究对象,分别对其开展经颅多普勒超声、彩色多普勒超声与血管造影检查,以患者的血管造影检查结果为诊断金标准来判定实际病变情况,依次观察并比较单纯经颅多普勒超声、单纯彩色多普勒超声与联合检查的诊断效果。结果经颅多普勒超声、彩色多普勒超声与联合检查分别检出动脉血管病变82例、86例、94例,三种检查方法与血管造影检查的符合率分别为82.9%、88.2%、98.0%,联合检查的诊断符合率明显高于单纯一种检查方式,具有统计学意义(P<0.05)。同时,联合检查的阳性预测值、阴性预测值、特异性与敏感度均明显高于单纯一种检查方式,具有统计学意义(P<0.05)。结论采用彩色多普勒超声联合经颅多普勒超声检查方式可提高颈部血管狭窄的诊断准确率,具有确切临床应用价值。Objective To analyze the clinical effect of color Doppler ultrasound combined with transcranial Doppler ultrasound in the diagnosis of neck vascular stenosis.Methods 76 patients with suspected cervical vascular stenosis in our hospital from January to December 2019 were selected as the research objects.Transcranial Doppler ultrasound,color Doppler ultrasound and angiography were performed respectively.The results of angiography were used as the diagnostic gold standard to determine the actual lesions.The diagnostic effects of single transcranial Doppler ultrasound,single color Doppler ultrasound and combined examination were observed and compared in turn.Results 82 cases,86 cases and 94 cases were detected by transcranial Doppler,color Doppler and combined examination respectively.The coincidence rates of three examination methods and angiography were 82.9%、88.2%and 98.0%respectively.The coincidence rate of combined examination was significantly higher than that of single examination,which was statistically significant(P<0.05).At the same time,the positive predictive value,negative predictive value,specificity and sensitivity of the combined test were significantly higher than that of the single test(P<0.05).Conclusion Color Doppler ultrasound combined with transcranial Doppler ultrasound can improve the accuracy of the diagnosis of cervical vascular stenosis,which has a definite clinical application value.
