摘　　要：目的探讨综合医院临床护士胰岛素规范注射的现状及影响因素,进行以问题为导向的胰岛素规范注射培训,旨在提高注射管理质量。方法2019年1月对某三级甲等综合医院1425名护士进行胰岛素规范注射相关知识、信念及行为的问卷调查,采用描述性分析、最优尺度回归分析等统计学方法对全部调查数据进行分析,调查综合医院临床护士胰岛素规范注射的现状及影响因素;对以问题为导向的胰岛素规范注射培训模式进行效果评价。结果临床护士胰岛素规范注射相关知识、信念及行为总体水平偏低,尤其是胰岛素规范注射相关知识掌握较差。最优尺度回归分析显示学历、工作年限、是否进行过胰岛素注射技术教育、近1年内接受胰岛素规范注射技术培训的次数、接受过几种形式的胰岛素规范注射技术培训是临床护士胰岛素规范注射相关知识的影响因素。基于问题为导向的综合医院胰岛素规范注射培训模式能提高临床护士胰岛素规范注射认知水平、技能水平及全院胰岛素规范注射护理质量。结论护理管理者应加强对低学历、低职称、低工作年限护士胰岛素规范注射相关知识、信念、行为的培训,并增加培训次数、培训形式,从而提高其对胰岛素规范注射相关知识的掌握水平,规范临床护士自身及患者的胰岛素注射技术,最终维护和促进糖尿病患者的健康。Objective To explore the status and influencing factors of standardized insulin injections in clinical nurses of general hospitals,and to conduct problem-oriented standardized insulin injection training in order to improve the quality of injection management.Methods In January 2019,a questionnaire survey was conducted on 1,425 nurses in a third-class general hospital for the knowledge,belief and behavior of standardized insulin injection,and statistical methods such as descriptive analysis and optimal scale regression analysis were used for all survey data to analyze and investigate the status and influencing factors of standardized insulin injections for clinical nurses in general hospitals,and evaluate the effect of the problem-oriented standardized insulin injection training model.Results The clinical nurses had a relatively low level of knowledge,beliefs and behaviors related to standardized insulin injection,especially poor knowledge of standardized insulin injection.Optimal scale regression analysis showed that the education,working years,whether or not have received technical education of insulin injection,the number of technical training of standardized insulin injection received in the past year,and received several forms of technical training of standardized insulin injection were the influencing factors of clinical nurses'standardized insulin injection related knowledge.The problem-oriented standardized insulin injection training model in general hospitals can improve the knowledge level of clinical nurses'standardized insulin injection,skill level and hospital-wide standardized insulin injection care quality.Conclusion Nursing managers should strengthen the training of nurses with low education,low professional title,and low working years on the knowledge,beliefs,and behaviors of standardized insulin injection,and increase the number of trainings and training forms,so as to improve their mastery of insulin injection related knowledge,standardize the insulin injection technology of clinical nurses and
