质量控制管理在急性缺血性脑卒中溶栓绿色通道建设中的应用

摘　　要：目的探讨质量控制管理在急性缺血性脑卒中(AIS)静脉溶栓绿色通道建设中的应用效果。方法选择2018年5—12月接受rt-PA溶栓治疗的AIS患者56例作为试验组,通过急性脑卒中绿色通道质控管理,选择2017年9月至2018年4月接受rt-PA溶栓治疗的AIS患者56例作为对照组,按照传统流程管理,比较两组患者进入医院到静脉溶栓开始给药时间(DNT)及DNT达标率、治疗前后神经功能缺损(NIHSS)评分、疗效和并发症发生率。结果试验组AIS患者rt-PA静脉溶栓DNT短于对照组,DNT达标率高于对照组,差异有统计学意义(P<0.05);两组NIHSS评分、溶栓疗效、并发症、病死率比较差异无统计学意义(P>0.05)。结论急性脑卒中绿色通道建设的质量控制管理,对流程优化有重要作用,可缩短DNT,但本研究未能观察到可以提高治疗效果,具有一定临床价值。Objective To explore the application effect of quality control management in the construction of green channel for acute ischemic stroke(AIS)intravenous thrombolysis.Methods To select 56 cases of patients with AIS who received rt-PA thrombolytic therapy from May to December 2018 as a test group,and given them quality control management of acute stroke green channel.Another 56 cases of patients with AIS who received rt-PA thrombolytic therapy from September 2017 to April 2018 were served as a control group,and they were managed according to traditional procedures.The door to needle time(DNT),control rate of DNT,neurological deficit(NIHSS)score before and after treatment,efficacy and incidence rate of complications of patients in the two groups.Results The DNT of rt-PA intravenous thrombolysis of AIS patients in the test group was shorter than that of the control group,and the control rate of DNT was higher than that of the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in NIHSS score,thrombolysis,complications,and mortality rate between the two groups before the management(P>0.05).Conclusion The quality control management of green channel construction in acute stroke had an important role in optimizing the process and can shorten DNT,but this study failed to observe that it can improve the treatment effect,it had certain clinical value.
