摘　　要：目的:研究颅内星形细胞瘤不同分级的磁共振波谱(MRS)影像诊断与病理检查的相关性。方法:选取医院接治的52例星形细胞瘤患者的MRS影像检查资料,对52例患者的病理检查数据和MRS影像数据进行分析。按照世界卫生组织(WHO)脑肿瘤分类法的分级(Ⅰ~Ⅳ)标准,分别对影像诊断和病理检查结果进行评分,并测量MRS波谱图中为胆碱与肌酸比值(Cho/Cr)、胆碱与乙酰天门冬氨酸比值(Cho/NAA)和乙酰天门冬氨酸与肌酸比值(NAA/Cr)。结果:Spearman相关分析,星形细胞瘤肿瘤区代谢物Cho/Cr、Cho/NAA和NAA/Cr以及MRS影像诊断结果均与病理检查结果高度相关(r=0.815,r=0.889,r=0.870,r=0.814;P<0.01)。结论:MRS影像分析数据能够较准确地诊断星形细胞瘤的分级,在星形细胞瘤的临床诊断和治疗效果观察中具有重要的应用价值。Objective:To discuss the correlation of magnetic resonance spectroscopy(MRS)imaging diagnosis and pathological examination for different grading of cerebral astrocytoma.Methods:MRS imaging data of 52 patients with astrocytoma who admitted in hospital were selected,and their pathological examination data were compared with their MRS imaging data.According to the grading standard(I-IV)of classification method of cerebral tumor of world health organization(WHO),the results of imaging diagnosis and pathological examination were scored respectively,and the ratios of choline and creatine(Cho/Cr),choline and N-acetylaspartate(Cho/NAA)and N-acetylaspartate and creatine(NAA/Cr)in MRS spectrum were measured.Results:The results of Spearman correlation analysis indicated that ratios(Cho/Cr,Cho/NAA and NAA/Cr)of metabolite and the results of MRS imaging diagnosis were highly correlated with the results of pathological examination(r=0.815,r=0.889,r=0.870,r=0.814,P<0.01).Conclusion:The analysis data of MRS image can accurately diagnose the grading of astrocytoma,and it has important application value in clinical diagnosis of astrocytoma and observation on therapeutic effect.
