摘  要:目的:研究磁共振定量非对称回波的最小二乘估算法迭代水脂分离序列(IDEAL-IQ)技术对脊髓型颈椎病脊髓损伤早期诊断的敏感性。方法:选取在医院住院治疗的41例脊髓型颈椎病患者,将其纳入观察组,另选在医院门诊检查的50名无颈脊髓压迫的健康者,将其纳入对照组;分别对两组进行磁共振成像(MRI)T2WI、IDEAL-IQ成像,分别在脂肪分数图和弛豫率(R2^*)图测算脊髓兴趣区脂肪分数值(FF)和R2^*值。结果:两组测得的FF值和R2^*值比较,差异有统计学意义(t=8.172,t=6.178;P<0.05)。观察组患者中脊髓受压部位脂肪含量改变为38例(占92.7%),T2WI脊髓内可见高信号21例(占51.2%),IDEAL-IQ技术比常规MRI对脊髓损伤检测更敏感。结论:IDEAL-IQ技术可以提供脊髓的病理变化信息,在早期诊断脊髓型颈椎病的脊髓损伤更有价值。Objective:To explore the sensitivity of decomposition of water and fat with echo asymmetry and leastsquares estimation quantitation sequence(IDEAL-IQ)technique of magnetic resonance(MR)in early diagnosis for spinal cord injury of cervical spondylotic myelopathy.Methods:41 patients with cervical spondylotic myelopathy who admitted to hospital were selected and were included in observation group,and 50 healthy persons without cervical spinal cord compression that received examination in outpatient department of hospital were included in control group.And magnetic resonance imaging(MRI)T2WI and IDEAL-IQ imaging were used to measure the fat fraction(FF)value and R2^*value of the region of interest of spinal cord in the fat fraction graph and relaxation rate(R2^*)graph,respectively.Results:The differences of FF values and R2^*values of the two groups were significant(t=8.172,t=6.178,P<0.05).In the observation group,the content of fat of the compression site of spinal cord of 38 patients(92.7%)was changed,and 21 cases(51.2%)occurred high signal of spinal cord in T2WI.IDEAL-IQ technique is more sensitive than routine MRI in detecting spinal cord injury.Conclusion:IDEAL-IQ technique can provide information about the pathological changes of the spinal cord,which is more valuable in the early diagnosis of spinal cord injury of cervical spondylotic myelopathy.
