摘  要:目的:研究彩色多普勒超声产前诊断胎儿畸形的影像学特征,为产妇产前诊断胎儿畸形、指导优生遗传提供依据。方法:选取医院行产前检查的8742名产妇,分别采用二维超声和三维彩色多普勒超声(三维超声)诊断,以超声声像图特征为依据,分析产妇产前二维超声和三维超声诊断胎儿畸形的临床应用与超声声像特征分析及其对优生遗传的指导价值。结果:在受检的8742名产妇中,随访结果证实274例胎儿畸形,其畸形胎儿的畸形部位为103处。在产前诊断中二维超声检出体表畸形90处,三维超声检出体表畸形101处;二维超声检查疑、漏诊13处,产前三维超声检查疑、漏诊2处,三维超声对体表畸形的诊断正确率明显高于二维超声,其差异有统计学意义(x^2=11.842,P<0.05)。结论:产前三维彩色多普勒超声可显示胎儿头面部畸形、心脏畸形、骨性结构畸形以及中枢神经系统畸形等影像学特征,为医师向产妇指导优生遗传提供依据。Objective:To investigate the imaging characteristics of prenatal diagnosis of fetal malformation by color Doppler ultrasound so as to provide references for prenatal diagnosis of fetal malformation and for guiding eugenic heredity.Methods:This study selected 8742 parturients who underwent prenatal examinations in hospital.The two-dimensional ultrasonography and three-dimensional color Doppler ultrasound were adopted,respectively.And the ultrasonogram characteristics was used as reference to analyze the clinical application and the analysis of ultrasonogram characteristics of two-dimensional ultrasonography and three-dimensional color Doppler ultrasound in diagnosing fetal malformation,and to analyze guiding value of them for eugenic heredity.Results:In 8742 parturients who were detected,the results of follow-up survey indicated that 274 fetuses were fetal malformations,and there were 103 malformations positions in 274 fetuses.In prenatal diagnosis,90 malformations in body surface were found by two-dimensional ultrasound,and 101 malformations in body surface were found by three-dimensional ultrasound.13 cases were suspected or missed diagnosis by two-dimensional ultrasound,and 2 cases were suspected or missed by three-dimensional ultrasound.For the malformation of body surface,the diagnostically accuracy rate of three-dimensional ultrasound was significantly higher than that of two-dimensional ultrasound(x^2=11.842,P<0.05).Conclusion:Prenatal color Doppler ultrasound can show imaging features such as fetal head and face malformation,cardiac malformation,bony structural malformation and central nervous system malformation.And it can provide basis of guiding eugenic heredity for physicians to parturient.
