超声在肺癌锁骨上淋巴结转移诊断中的应用

摘　　要：目的评价超声检查在肺癌锁骨上淋巴结转移中的临床应用价值。方法选取我院收治的130例肺癌并锁骨上淋巴结转移确诊患者作为研究对象,均给予颈部超声检查,按照转移淋巴结是否典型分为A组(典型组)76例与B组(非典型组)54例,对两组患者进行超声引导下穿刺细胞学检查(FNAC),比较两组患者临床分析结果。结果A组患者腺癌发生率明显高于B组(χ^2=17.291,P<0.01),A组FNAC阴性率明显低于B组(χ^2=13.529,P<0.01);两组患者鳞癌、小细胞癌、未分化癌及结核性淋巴结肿大、炎性淋巴结肿大、恶性淋巴瘤等发生率比较并无明显差异(P>0.05);彩色多普勒超声血流显像(CDFI)检测与FNAC对肺癌锁骨上淋巴结转移诊断准确性类似,但CDFI特异度高于FNAC(χ^2=8.265,P<0.05),假阴性率低于FNAC(χ^2=7.355,P<0.05),具有统计学意义。结论超声检查可用于肺癌锁骨上淋巴结转移临床筛查,尤其对腺癌的诊断准确性较高。Objective:To evaluate the value of ultrasonography in the clinical application of supraclavicular lymph node metastasis of lung cancer.Method:Choose 130 cases of our hospital and supraclavicular lymph node metastasis of lung cancer diagnosed patients as the research object,have given the neck ultrasound examination,according to the lymph nodes metastasis is typically divided into group A(typical)76 cases and group B(atypical group)54 cases,the two groups of patients with ultrasound guided biopsy cytology(FNAC),compared two groups of patients with clinical analysis results.Results:The incidence of adenocarcinoma in group A was significantly higher than that in group B(χ^2=17.291,P<0.01),and the FNAC negative rate in group A was significantly lower than that in group B(χ^2=13.529,P<0.01).There was no significant difference in the incidence of squamous cell carcinoma,small cell carcinoma,undifferentiated carcinoma,tuberculous lymph node enlargement,inflammatory lymph node enlargement and malignant lymphoma between the two groups(P>0.05).CDFI was similar to FNAC in the diagnosis of supraclavicular lymph node metastasis of lung cancer,but CDFI was more specific than FNAC(χ^2=8.265,P<0.05),and the false negative rate was lower than that of FNAC(χ^2=7.355,P<0.05).Conclusion:Ultrasonography can be used for clinical screening of lung cancer with supraclavicular lymph node metastasis,especially for adenocarcinoma.
