尿NGAL、β2-MG检测对2型糖尿病早期肾损伤的诊断价值

摘　　要：目的 探讨尿NGAL、β2-MG检测对2型糖尿病早期肾损伤的诊断价值。方法 选取我院2019年5月~2019年10月收治的2型糖尿病患者120例,根据UACR水平分为对照组(UACR≤30mg/24h),MA组[微量白蛋白尿(UACR≥30~300mg/24h)],CA组[临床白蛋白尿(UACR≥300mg/24h)],每组各40例患者。采用放射免疫分析法分析NGAL和β2-MG与肾小球滤过率(GFR)的相关性。应用(ROC曲线)分析NGAL和β2-MG对糖尿病患者早期肾损伤诊断的敏感性与特异性。结果 尿液中NGAL、β2-MG水平随UACR水平的增加而增加,组间比较有显著性差异(P<0.05);β2-MG水平CA组与MA组比较亦有显著性差异(P<0.05);MA组NGAL和β2-MG水平与GFR成明显的负相关(r=-0.791和r=-0.738),尿中NGAL和β2-MG的ROC曲线下面积分别为0.903和0.833。结论 尿中NGAL和β2-MG均可作为糖尿病早期肾损伤的诊断指标,但在敏感性与特异性上NGAL更优于β2-MG。Objective:To investigate the clinical value of NGAL andβ2-MG in the diagnosis of renal injury in patients with type 2 diabetes.Methods:Totally,120 patients with type 2 diabetes were admitted and treated in our hospital from May 2019 to October 2019.According to UACR,they were divided into control group(UACR≤30 mg/24 h),MA group(UACR≥30~300 mg/24 h),and CA group(UACR≥300 mg/24 h)with 40 patients for each.The correlation between NGAL/β2-MG and GFR was analyzed by radioimmunoassay.The diagnostic sensitivity and specificity of NGAL andβ2-MG for renal injury in patients with type 2 diabetes were detected by ROC curve.Results:The levels of NGAL andβ2-MG increased with the increase of UACR.There was a significant difference inβ2-MG between CA group and MA group(P<0.05).In MA group,the levels of NGAL andβ2-MG negatively correlated with GFR(r=-0.791,r=-0.738).The area under curve of NGAL andβ2-MG were 0.903 and 0.833.Conclusions:It is shown that NGAL is superior toβ2-MG,although both of them can be used in the diagnosis of renal injury in patients with type 2 diabetes.
