内镜下鼻骨骨折复位联合鼻中隔成形术对鼻通气功能及美学效果的影响研究

摘　　要：目的:探讨内镜下鼻骨骨折复位联合鼻中隔成形术对鼻通气功能及美学效果的影响,为鼻骨骨折的治疗提供参考。方法:选取行手术治疗的鼻骨骨折患者80例,随机分为对照组与观察组,每组40例,分别行内镜下鼻骨骨折复位联合鼻中隔矫正术及内镜下鼻骨骨折复位联合鼻中隔成形术治疗,观察术前及术后6个月时的鼻腔通气功能变化及鼻部外形美学效果。结果:两组术前鼻腔通气评分、鼻腔容积、鼻腔最小横截面积、鼻腔呼气吸气阻力比较差异无统计学意义(P>0.05);两组术后均较术前明显改善,且观察组术后6个月改善程度优于对照组,差异有统计学意义(P<0.05)。两组患者术前鼻部美学效果评分比较差异无统计学意义(P>0.05),两组术后6个月较术前明显改善(P<0.001),但组间比较差异无统计学意义(P>0.05)。对照组治疗优良率为87.50%,观察组为95.00%,组间比较差异无统计学意义(P>0.05)。结论:内镜下鼻骨骨折复位术结合鼻中隔成形术治疗鼻骨骨折伴鼻中隔骨折改善鼻通气功能更为明显,疗效及美学效果与联合鼻中隔矫正术相当。Objective To investigate effect on the influence of nasal ventilation function and aesthetic in endoscopic reduction of nasal bone fracture with septoplasty,to provide reference for the treatment of nasal fracture.Methods The 80 cases of nasal bone fracture were treated by operation,randomly divided into the control group and the observation group,each group of 40 cases,endoscopic reduction of nasal bone fracture combined with nasal septal correction and endoscopic reduction of nasal bone fracture combined with nasal septoplasty were performed respectively.The changes of nasal ventilation function and the aesthetic effect of nasal shape before and 6 months after operation were observed.Results Preoperative VAS score of nasal ventilation,nasal volume,nasal minimum cross-sectional area,nasal expiratory and inspiratory resistance,there was no statistically significant difference between the two groups(P>0.05).Both groups improved significantly after operation,and the improvement degree of the observation group were better than those of the control group,the differences were statistically significant(P<0.05).There was no significant difference between the two groups in the score of aesthetic effect of nose before operation(P>0.05).Six months after the operation,the two groups were significantly improved compared with that before the operation(P<0.001).But there was no significant difference between the two groups(P>0.05).The excellent and good rate of the control group was 87.50%,and that of the observation group was 95.00%,there was no statistically significant difference between the two groups(P>0.05).Conclusion The improvement of nasal ventilation function is more obvious in endoscopic reduction of nasal bone fracture with septoplasty,the therapeutic effect and aesthetic effect are equivalent to the combined nasal septum correction.
