应用整形外科技术急诊修复严重鼻外伤20例

摘　　要：目的:探讨整形外科修复原则在鼻损伤早期治疗中的意义。方法:对鼻部损伤患者判定伤情后,应用整形外科无创、无菌及避免直线缝合等原则,对位离断的鼻组织,分黏膜层、软骨层及皮肤等层次,按照鼻部亚单位美学标准逐一缝合修复。结果:本组20例患者,鼻部单纯皮肤软组织裂伤缝合术7例,鼻部立体三维裂伤修复3例,皮肤软组织缺损行鼻唇沟皮瓣修复3例,植皮修复6例,耳复合组织块移植修复1例。20例患者中除1例皮瓣远端坏死后清创换药愈合,1例鼻翼回植后局部坏死换药瘢痕愈合二期行耳复合组织移植修复,其余患者伤口均一期愈合。随访3~6个月,6例植皮患者局部有轻度色素沉着,无明显瘢痕增生、挛缩等并发症,外形及功能恢复良好。结论:应用整形外科修复原则行急诊修复面部及鼻亚单位损伤具有较好的临床价值。Objective To explore the significance of plastic surgery repair principles in the early treatment of nasal injury.Methods After the injury was determined in patients with nasal injury,the principles of non-invasive,sterile and avoiding straight suture were applied.The nasal tissue,the mucosal layer,the cartilage layer and the skin were separated and repaired according to the aesthetic criteria of the nasal subunit.Results 20 patients were treated.There were 7 cases of nasal soft tissue laceration and suture in the nose,3 cases of stereoscopic three-dimensional laceration in the nose,3 cases of repair of nasolabial flap with skin soft tissue defect,6 cases of skin graft repair,and one case of ear tissue graft.In 20 patients,except for one case of distal flap necrosis,the debridement dressing was healed.In one case,the local necrosis dressing scar healing was performed after the nasal replantation.The other patients were healed by the first stage.The patients were followed up for 3-6 months,6 patients with skin grafts had mild hyperpigmentation,no obvious complications such as scar hyperplasia and contracture.The skin color was basically the same,and the shape and function recovered well.Conclusion The application of plastic surgery repair principle has a good clinical value in emergency repair of facial and nasal subunit injuries.
