灌注指数监测对经桡动脉行CAG术后穿刺影响的护理研究

摘　　要：目的评价在灌注指数(PI值)监测下对经桡动脉行冠脉造影(CAG)术后穿刺处的护理。方法选择2017年07月~2018年11月间我院心内科收治的100例经桡动脉行CAG冠心病患者,按照数字表随机分为对照组和观察组,每组50例。对照组术后2h、4h、6h(每2h)松解加压包扎的弹力绷带,观察组参照精准PI值监测数值给予及时适当调整术肢高度,保持PI灌注指数为1.4及以上,可直至术后6h。对比两组患者术后24h穿刺处疼痛、皮肤水泡、出血、血肿发生情况,以及术后3d有无血管闭塞。结果两组患者的疼痛、皮肤水泡、出血、血肿及血管闭塞并发症的发生率有明显差异,观察组发生率明显低于对照组,差异具有统计学意义(P<0.05)。结论PI值可精准明确冠心病患者CAG穿刺处压迫情况,为临床护理人员及时调整患者术肢提供客观参照数据,减轻患者术后并发症的发生,减轻临床护理工作量。Objective To evaluate the nursing of the puncture site after coronary artery angiography(CAG)in the radial artery under the monitoring of perfusion index(PI value).Methods 100 patients with CAG coronary heart disease who underwent radial artery admission in our hospital from July 2017 to November 2018 were randomly divided into control group and observation group according to the digital table,50 cases in each group.In the control group,the elastic bandage was loosened at 2h,4h,6h(every 2h).The observation group was given the accurate PI value to adjust the height of the limb in time,and the PI perfusion index was 1.4 and above.After 6h.The pain,skin blisters,hemorrhage,hematoma occurred at 24 hours postoperatively,and vascular occlusion was observed 3 days after surgery.Results The incidence of pain,skin blisters,hemorrhage,hematoma and vascular occlusion complications were significantly different between the two groups.The incidence of the observation group was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion PI value can accurately determine the compression of CAG puncture in patients with coronary heart disease,provide objective reference data for clinical nurses to adjust the limbs of patients in time,reduce the incidence of postoperative complications and reduce the workload of clinical nursing.
