经皮射频微创治疗伴深静脉瓣膜功能不全的下肢静脉曲张患者的临床疗效

摘　　要：目的探讨经皮射频微创治疗伴深静脉瓣膜功能不全(DVI)的下肢静脉曲张患者的临床疗效。方法选取中国医学科学院北京协和医院2018年01~12月收治的59例经皮射频微创治疗的伴有DVI的下肢静脉曲张患者。术后3~5天统计患者临床疗效及并发症发生情况,术后12个月统计患者患肢深静脉通畅及瓣膜功能情况。术前、术后12个月采用慢性静脉功能不全生活质量调查表(CIVIQ)和静脉临床严重程度评分表(VCSS)评估患者生活质量。结果术后3~5天复查,59例患者经皮射频微创治疗的静脉主干均获得有效闭合。术后12个月,大隐静脉主干再通1例,病理性穿支静脉再通3例;1例患者新发肌间静脉血栓;深静脉瓣膜返流时间较术前减少21例,59例患者术前的深静脉瓣膜返流时间(4.48±1.72)s,而术后12个月为(3.94±1.85)s,差异有统计学意义(P<0.05)。术后12个月CIVIQ和VCSS评分与术前比较,差异有统计学意义(P<0.01)。结论经皮射频微创治疗伴有DVI的下肢静脉曲张的患者,可获得满意的1年期疗效,且深静脉瓣膜返流时间减少。Objective To explore the clinical effect of radiofrequency ablation for varicose veins of lower extremities with deep vein insufficient(DVI).Method From January to December 2018,59 patients with varicose veins of lower extremities with DVI were enrolled in Peking Union Medical College Hospital.The clinical effect and complications of the patients were counted 3-5 days after operation,and the patency of deep vein and the function of valve were counted 12 months after operation.The quality of life was assessed by chronic venous insuficiency quality of life questionnaire(CIVIQ)and venous clinical severity score(VCSS)before and 12 months after operation.Result At 3-5 days after the operation,the main veins of 59 patients were effectively closed.12 months after operation,the main saphenous vein was reopened in 1 case,incompetent perforator vein was reopened in 3 cases,new intermuscular vein thrombosis was found in 1 case,the time of deep vein valve regurgitation was decreased in 21 cases,59 cases(4.48±1.72)s,and 12 months after operation(3.94±1.85)s,the difference was statistically significant(P<0.05).There was significant difference in CIVIQ and VCSS scores 12 months after operation(P<0.01).Conclusion Radiofrequency ablation for lower extremities varicose veins combined with DVI has satisfactory effect at 1 year postoperative,and the time of deep vein valve regurgitation is reduced.
