摘  要:目的探讨外周血免疫及炎性指标与下肢动脉硬化严重程度相关性分析。方法选取2019年1月至2019年8月就诊于安徽医科大学附属省立医院下肢动脉硬化症(LEAD)患者45例,按照临床症状严重程度分为间歇性跛行组(n=24)和严重肢体缺血组(n=21),收集两组患者一般资料及外周血指标进行统计分析。结果间歇性跛行组患者外周血调节性T细胞(Treg)/CD4+T细胞、调节性B细胞(Breg)/B细胞均高于严重肢体缺血组患者,中性粒细胞与淋巴细胞比值(NLR)低于严重肢体缺血组患者,差异有统计学意义(P<0.05);NLR与LEAD患者Rutherford分级呈正相关,Breg/B细胞与LEAD患者Rutherford分级呈负相关;NLR为LEAD患者严重肢体缺血的独立危险因素。结论免疫功能失调及炎性反应加重导致了下肢动脉硬化的进展。Objective To explore the correlation between the immune and inflammatory indexes of peripheral blood and the severity of lower extremity atherosclerosis.Method From January 2019 to August 2019,45 patients with lower extremity atherosclerosis diseases(LEAD)were selected from the Provincial Hospital Affiliated to Anhui Medical University.According to the severity of clinical symptoms,they were divided into intermittent claudication group(n=24)and severe limb ischemia group(n=21).The general data and peripheral blood indexes of the two groups were collected for statistical analysis.Result The proportion of regulatory T cells(Treg)in CD4+cells and regulatory B cells(Breg)in B cells in peripheral blood of patients with intermittent claudication was higher than that of patients with severe limb ischemia,and the neutrophil to lymphocyte ratio(NLR)was lower than that of patients with severe limb ischemia,the difference was statistically significant;NLR was positively correlated with Rutherford grade of patients with LEAD,and the percentage of Breg in B cells was negatively correlated with Rutherford grade of patients with LEAD;NLR was negatively correlated with Rutherford grade of patients with LEAD it is an independent risk factor for severe limb ischemia in patients with lower extremity arteriosclerosis.Conclusion The development of arteriosclerosis in the lower extremity was caused by the immune dysfunction and the aggravation of inflammatory reaction.
