LEEP联合重组人干扰素α-2b凝胶治疗宫颈上皮内瘤变的疗效及对患者免疫功能的影响

摘　　要：目的评价宫颈上皮内瘤变患者采用宫颈环形电切术(LEEP)联合重组人干扰素α-2b凝胶治疗的疗效及对其免疫功能的影响。方法根据随机对照原则选取2017年2月至2018年2月深圳市龙岗区妇幼保健院诊治的104例宫颈上皮内瘤变患者作为研究对象。分为研究组(n=52)和对照组(n=52)。研究组采用LEEP联合重组人干扰素α-2b凝胶治疗,对照组仅采用LEEP治疗,疗程均为10d。评估两组临床疗效、治疗相关指标、12个月后复发率及并发症,并观察治疗前后T淋巴亚群、炎症反应[白细胞介素-10(IL-10)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)]指标变化。结果研究组治疗总有效率(94.23%)较对照组(80.77%)高,12个月后复发率(1.92%)较对照组(15.38%)低,差异具有统计学意义(P<0.05);与对照组相比,研究组术中出血量较低,愈合时间、阴道流液时间、住院时间、腰骶酸痛消失时间、分泌物过多消失时间及血性白带消失时间均较短,差异具有统计学意义(P<0.05);研究组治疗10d后CD4^+、CD4^+/CD8^+、IL-10水平比对照组高,CD8^+、CRP、TNF-α水平比对照组低,差异具有统计学意义(P<0.05);研究组并发症发生率(7.69%)比对照组(17.31%)略低,但差异无统计学意义(P>0.05)。结论在LEEP基础上联合重组人干扰素α-2b凝胶治疗宫颈上皮内瘤变的疗效确切,可加快患者临床症状缓解,缩短整体治疗时间,提高机体免疫功能,降低炎性反应,且可减少围术期并发症。Objective To evaluate the efficacy of loop electrosurgical excision procedure(LEEP)combined with recombinant interferonα-2 b suppository in the treatment of cervical intraepithelial neoplasias(CIN)and its influence on the immune function.Methods 104 patients with CIN in Longgang Maternal and Child Health Hospital from February 2017 to February 2018 were divided into two groups by randomized controlled method.Research group(n=52)took LEEP and recombinant interferonα-2 b suppository,while control group(n=52)was treated with LEEP.The course of treatment was 10 d.The clinical efficacy,treatment-related indexes,recurrence rate after 12 months,complications in the two groups were evaluated.The T lymphocyte subsets,inflammatory response[interleukin-10(IL-10),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)]before and after treatment were observed.Results The total effective rate of research group(94.23%)was higher than that in the control group(80.77%),and the recurrence rate after 12 months in the research group(1.92%)was lower than that in the control group(15.38%)(P<0.05);Compared with the control group,the intraoperative bleeding volume in research group was lower,the healing time,vaginal discharge time,hospital stays,disappearance time of lumbosacral pain,disappearance time of hypersecretion and disappearance time of hemorrhagic leucorrhea in the research group were shorter(P<0.05);After 10 days of treatment,the levels of CD4^+,CD4^+/CD8^+,IL-10 in the research group were higher than those in the control group,while the levels of CD8^+,CRP,TNF-αin research group were lower than those in control group(P<0.05);The incidence of complications in research group(7.69%)was slightly lower than control group(17.31%),but there was no statistically significant differences(P>0.05).Conclusions Recombinantα-2 b interferon suppository combined with LEEP is effective in the treatment of CIN,which can relieve clinical symptoms,shorten treatment time,increase immune function,decrease inflammatory response,and reduce peri
